Manual de Emergencias enGastroenterología y HepatologíaINFECCIONES DEL TRACTO URINARIOCAUSAS EXTRADIGESTIVAS DE DAA1234567Las infecciones del tracto urinario (ITU) comprenden una ampliavariedad de patologías que tienen en común la colonizaciónbacteriana progresiva de tejidos y vías que se extienden desdeel meato uretral hasta la corteza renal. Son las infecciones másfrecuentes en nuestro medio tras las del aparato respiratorio.Se caracterizan por disuria, polaquiuria, micción urgente, y amenudo van acompañadas de dolor suprapúbico, orina malolientey, en ocasiones, hematuria.Las ITU complicadas son definidas como aquellas infeccionesurinarias asociadas, bien a anormalidades (estructurales o funcionales)del tracto genitourinario, o bien a enfermedades subyacentesque aumentan el riesgo de la infección o el fallo terapéutico.Algunas circunstancias comportan un mayor riesgo de ITU complicada(tabla I).Pauta antibiótica corta (tabla II).Los criterios para la indicación de una ecografía renal son:• Fiebre persistente de más de 72 h de evolución, a pesar detratamiento antibiótico adecuado.• Historia de ITU recurrentes.• Clínica atípica por hematuria persistente o dolor cólico.• Coexistencia de embarazo.• Presencia de anormalidades estructurales y/o antecedentesde litiasis renal.Ante una sepsis con posible obstrucción del drenaje urinario laactitud a seguir es antibioticoterapia intravenosa (AB iv) y derivaciónurinaria urgente mediante cateterismo ureteral o nefrostomíapercutánea (NPC).176
