Durante el procedimiento del TIPS, se recomienda iniciar una perfusión deheparina sódica inmediatamente después de la punción de la vena portapara evitar la trombosis precoz de la prótesis.2. La trombosis y la disfunción del TIPS es menos frecuente desde la introducciónde prótesis recubiertas de politetrafluoroetileno. Por ello éstas son las prótesisque deben ser utilizadas de primera intención.3. La colocación del TIPS requiere una formación especializada y puede sermucho más compleja que en los pacientes con cirrosis hepática.4. En más del 45% de los casos, puede ser necesaria la realización de unapunción portal directamente desde la vena cava inferior por la presenciade una trombosis completa de las venas suprahepáticas.5. Por todo ello, estos pacientes deben ser referidos a centros con experiencia.6. El tratamiento previo con un TIPS no empeora el pronóstico en los pacientesque van a precisar un trasplante hepático cuando fracasa el procedimientoanterior.BIBLIOGRAFÍA1. Valla DC.
