A largo plazo el riesgode desarrollar carcinoma escamoso es 1.000-3.000 veces superioral de la población normal.6EMERGENCIAS ESOFÁGICAS1234La utilización de corticoides como medida preventiva de laestenosis se basa en su capacidad inhibitoria de fibrogénesis. Sinembargo, los metaanálisis llevados a cabo no han proporcionadoevidencias en favor de su eficacia. Además, su utilizacióncomporta probablemente un mayor riesgo de infección y perforación.Aunque existe controversia, algunos casos seleccionadospodrían beneficiarse de corticoides i.v (siempre con coberturaantibiótica). Ello incluiría lesiones grado IIb, especialmentelas provocadas por álcalis (> riesgo de estenosis). Las dosis nodeberían ser superiores a 1-2 mg/kg.Estudios retrospectivos no controlados recomiendan la colocaciónde un stent plástico o una SNG bajo visión endoscópicadirecta para prevenir el desarrollo de estenosis esofágica; además,en caso de desarrollarla, se describe una menor necesidadde dilataciones endoscópicas posteriormente. Sin embargo, suutilidad a largo plazo es controvertida por la alta tendencia a lamigración de los stents.Una vez establecida la estenosis el tratamiento de elección es ladilatación endoscópica mediante balón o bujías (a partir de las3 semanas para disminuir el riesgo de perforación). Algunas evidenciassugieren que la inyección local de corticoides (triamcinolona)proporciona un beneficio adicional mejorando los resultados.En otros casos puede ser útil la colocación de una prótesisplástica como paso previo a la cirugía.La estenosis gástrica es mucho menos frecuente que la esofágica,pudiendo requerir para su tratamiento dilataciones neumáticasy, en caso de fracaso, cirugía.
