Manual de Emergencias enGastroenterología y HepatologíaEFECTOS DE LA HTP SOBRE EL RIÑÓNEMERGENCIAS EN HEPATOLOGÍA123En las fases iniciales de la cirrosis, la función circulatoria se compensapor períodos transitorios de retención renal de sodio yagua que causan hipervolemia y aumento del gasto cardíaco.En una fase más avanzada, la vasodilatación esplácnica seacentúa y estos mecanismos resultan insuficientes para mantenerla homeostasis circulatoria, conduciendo a la hipotensiónarterial y a la estimulación de sistemas vasoactivos (SRAA, SNSy producción de ADH). Estos sistemas, que son potentes vasoconstrictores,incrementan la presión arterial hasta niveles normaleso casi normales, pero producen retención de sodio yagua, que conlleva el desarrollo de ascitis y edemas.Un tiempo después, el exceso de ADH determina una disminucióndel aclaramiento de agua libre (cantidad de agua librede soluto que el riñón es capaz de depurar por unidad de tiempo).Ello comporta una inadecuada retención de agua, proporcionalmentemuy superior a la de sodio, lo que explica laaparición hiponatremia dilucional.En un grado más avanzado, la vasoconstricción arteriolar renalinducida por los sistemas vasoactivos descritos provoca isquemiarenal. Durante un tiempo, el riñón es capaz de compensaresta situación, incrementando la producción de sustanciasvasodilatadoras (p.e. prostaglandinas, ON), lo que permitemantener una función renal normal. Sin embargo, a medidaque se agrava el trastorno hemodinámico, aumenta la producciónde sustancias vasoconstrictoras, provocando intensavasoconstricción arteriolar, descenso del filtrado glomerular yfallo prerrenal. No es infrecuente que esta situación aparezcatras un factor precipitante (“acute on chronic”) que agrava elinfrallenado del compartimento vascular (hemorragia digestiva,depleción de volumen por diarrea o diuréticos) o acentúael daño necroinflamatorio hepático (transgresión enólica, cirugíareciente, infecciones).300
