Manual de Emergencias enGastroenterología y HepatologíaHEMORRAGIA GASTROINTESTINAL1 El diagnóstico de una HDAA se completa con la realización deuna endoscopia digestiva alta (EDA), cuyas sensibilidad y especificidadsuperan el 95%. La recomendación es que en los casosgraves se lleve a cabo en cuanto se consiga la estabilidadhemodinámica, y en el resto dentro de las 12 a 24 horas siguientes(de forma ideal antes de transcurridas 6 horas). Cuando noes posible alcanzar la estabilidad hemodinámica debe valorarsela cirugía urgente con eventual endoscopia intraoperatoria.2 La EDA puede estar precedida de la administración de eritromicina1 (dosis única de 3 mg/kg, 30 a 90 minutos antes de laendoscopia) para promover el vaciamiento gástrico de los restoshemáticos acumulados. Esta medida ha demostrado ser eficazpara mejorar la visibilidad, acortar el tiempo de exploracióny reducir la necesidad de una segunda endoscopia.34Con los resultados de la endoscopia se elaboran prediccionespronósticas más precisas que las obtenidas con parámetrosexclusivamente clínicos. La estratificación de una úlcera en funciónde la clasificación de Forrest es un buen factor predictor deresangrado (tabla II). El índice de Rockall, que reúne datos clínicosy endoscópicos, permite seleccionar a un subgrupo depacientes de bajo riesgo que no van a precisar hospitalización(tabla III).Otros factores con valor pronóstico, no incluidos en las clasificacionescitadas, son el tamaño de la úlcera y su localización. Undiámetro superior a 2 cm y una ubicación en la parte alta de lacurvatura menor gástrica o en la cara posterior del bulbo duodenal,se asocian a un incremento en la tasa de recidiva hemorrágicay a un mayor índice de fracaso de la terapéutica endoscópica.1La eritromicina libera motilina.56
