La resección quirúrgica debería asociarsea una pauta de anticoagulación, inicialmente medianteheparina i.v durante 7-14 días y posteriormente la administraciónde dicumarínicos durante 6 meses.Si durante la laparotomía, el cirujano advierte que existen extensasáreas de tejido necrótico no viable, las alternativas son elcierre de la cavidad abdominal o la resección extensa del intestinocon un programa de nutrición parenteral domiciliaria.Si el segmento isquémico es extenso, pero existen segmentosque pueden ser viables tras el restablecimiento del flujo vascular,resulta aconsejable extirpar únicamente el tejido claramentenecrótico e iniciar una terapia de “rescate”. Ésta se basa enla heparinización, la infusión intraarterial de vasodilatadores y lapropia trombectomía (si la luz de la VMS está totalmente ocluídapor el trombo). En un segundo tiempo puede revisarse denuevo la viabilidad del intestino remanente, procediendo a laexéresis de las áreas de necrosis evidente.El uso de agentes trombolíticos ha demostrado su utilidad únicamenteen casos aislados, por lo que actualmente debe considerarseen fase experimental. Existen informes aislados de suutilidad cuando se administran por vía anterógrada, a través dela AMS, retrógrada (yugular) o transhepática, por vía portal.AMS: arteria mesentérica superior.138
