Manual de Emergencias enGastroenterología y Hepatología1TRATAMIENTO DEL SINDROME HEPATORRENALEl tratamiento debe iniciarse lo antes posible. No se ha demostradoque el tratamiento mejore la supervivencia pero los pacientes en losque revierte la I.R. presentan una supervivencia más prolongadaque aquellos en los que no revierte. El tratamiento está particularmenteindicado en pacientes candidatos a T.O.H1. En los pacientesno candidatos a trasplante, el tratamiento debe ser individualizado.2El tratamiento de elección consiste en la administración de vasoconstrictoresy albúmina. El vasoconstrictor más utilizado es la terlipresina(1 mg/4-6 h ev) con albúmina 20% (1 g/kg seguido de 20-40g/día). Existe menos experiencia con noradrenalina en infusión continua(0,3-5 mg/h hasta aumentar la presión arterial media en 10mmHg) y con midodrina, un agonista alfa-adrenérgico activo porvía oral. La terlipresina es el único fármaco aprobado en nuestropaís para el síndrome hepatorrenal. El tratamiento con vasoconstrictoresdebe realizarse preferentemente en una UCI o de cuidadosintermedios, prestando particular atención a la posible aparición deefectos secundarios de tipo isquémico y edema pulmonar.EMERGENCIAS EN HEPATOLOGIA3456El efecto del tratamiento debe monitorizarse mediante la determinaciónde los niveles de creatinina sérica. Si ésta diminuye enmás de un 30% durante los 3 primeros días, el tratamiento semantiene hasta lograr niveles < 1,5 mg/dl y a ser posible hasta
