Manual de Emergencias enGastroenterología y HepatologíaVASODILATACIÓN ESPLÁCNICA Y DISFUNCIÓN CIRCULATORIA1El principal responsable de la vasodilatación arterial esplácnicaobservada en el enfermo con cirrosis hepática e hipertensiónportal es un incremento en la síntesis de óxido nítrico (ON) deorigen extrahepático. Diversos factores parecen implicados eneste mecanismo:EMERGENCIAS EN HEPATOLOGÍA29823a. La activación de eNOS (óxido nítrico sintetasa endotelial)derivada del estímulo físico que comporta el estrés de razomiento(shear stress) ligado al aumento de la resistencia alflujo sanguíneo.b. La activación de sNOS eNOS debida al estímulo de sistemasparacrinos locales (cannabinoides endógenos, monóxidode carbono), que están marcadamente incrementados enpacientes con cirrosis e HTP.c. Por otra parte, la traslocación bacteriana (migración debacterias procedentes de la luz intestinal hasta los ganglioslinfáticos mesentéricos) y su posterior incorporación a la circulacióngeneral favorece la liberación de citoquinas yendotoxinas que promueven la síntesis de ON del endoteliovascular.La vasodilatación esplácnica ocasiona cambios notables en lacirculación sistémica de los pacientes con cirrosis e HTP. Estoscambios consisten en una caída de las resistencias vascularesperiféricas y de la presión arterial, así como un aumento delgasto cardíaco y del volumen plasmático, parámetros que configuranla denominada circulación hiperdinámica del cirrótico.A su vez, la vasodilatación arterial causa una distribución anormaldel volumen plasmático, que conduce a una reduccióndel volumen central (corazón, pulmones y aorta). Este “infrallenado”del compartimento vascular es percibido por barorreceptoresarteriales y cardiopulmonares, como una hipovolemiaefectiva capaz de suscitar una respuesta neurohormonal conliberación de sustancias vasoactivas con propiedades vasoconstrictoras:sistema renina-angiotensina-aldosterona (SRAA),sistema nervioso simpático (SNS) y liberación de vasopresina uhormona antidiurética(ADH).
