El tratamiento inicial es la colocación de unaprótesis plástica con o sin esfinterotomía. La prótesis se mantiene2-3 meses hasta comprobar la cicatrización. Los casosrefractarios requieren cirugía.Los grandes bilomas que no se comunican con el árbol biliarrequieren un drenaje percutáneo y antibióticos.Los pacientes con defectos de llenado (cálculos, barro biliar,moldes, coágulos, migración de stent), son tributarios de esfinterotomía,al igual que la disfunción del esfínter de Oddi.Estenosis. El manejo difiere según la naturaleza de la estenosis.Deben diferenciarse las anastomóticas (que suelen ser aisladasy cortas) de las no anastomóticas (suelen ser múltiples y demayor longitud, se localizan en los conductos intrahepáticosy/o proximales a la anastomosis en el conducto del donante).a. Estenosis anastomóticas. El tratamiento de primeralínea es endoscópico ya que el abordaje percutáneoviene gravado por una mayor tasa de complicacionesy la cirugía se reserva para los enfermos que no respondena los tratamientos previos. El primer paso consisteen dilatar la estenosis con un balón de 6-8 mm.Seguidamente se procede a la colocación de variasprótesis de plástico de 7-11 Fr. Las prótesis tienen queser reemplazadas cada 3 meses durante un año. Eléxito de este tratamiento es del 70-100%. La estenosisinfranqueable o de aparición tardía (> 6 meses), la presenciade fístula biliar y el uso previo de tubo en "T" seasocian a una peor respuesta. Estos casos pueden responderbien a la cirugía (hepaticoyeyunostomía).b. Las estenosis no anastomóticas son más difíciles de tratarcon tasas de éxito del 50-75%. Se aconseja dilatacióncon balón de 4-6 mm y la colocación de un stentde 10-11,5 Fr. Por falta de respuesta, aproximadamenteun 50% de estos casos requerirán retrasplante. Lamortalidad en este subgrupo es elevada.Los pacientes con estenosis, cualquiera que sea su naturaleza,requieren de un estrecho seguimiento (vigilancia de enzimashepáticas, colangioRNM) dada la elevada tasa de recidivaobservada en estos casos.
