INTERVENCIONES PSICOLÓGICAS EN DUELO Y ENFERMEDAD ORGÁNICA

Intentaré transmitir en una apretada síntesis las
diferentes modalidades de abordaje puestas en
practica durante 10 años en un Hospital privado
de la ciudad de Buenos Aires, República Argentina.
Describiré la metodología de trabajo con niños
internados en pediatria , UTI pediátrica y neonatal.
Me detendré sucintamente en 3 casos de
adolescentes varones con peritonitis detectada
tardíamente, debido a lo confuso del cuadro
clínico, cuya vida estuvo en grave riesgo y que
transitaban por situaciones de duelo familiar.
Marcaré las diferencias con otros encuadres
psicoanalíticos presentando un caso de una joven
con anorexia grave que estuvo 3 meses internada
con 28 kilos sin responder al tratamiento siendo
tratada por psicoanalistas de renombre como
una histeria.
At Buenos Aires, Argentina, for many decades,
teams of professionals of mental health (psychologists
and psychiatrists) interact with general
doctors and specialists that work with patients
with organic pathologies.
During the decade of 1990’s, the medical
direction of a private clinic in Bs. As. decided to
hire psychologists and psychiatrists in order to
collaborate with the doctors in the attention of
patients. The collaboration consisted in a system
of interconsultations in which the doctor in
charge could ask for the intervention of a specialist
of mental health. It is important to note that
in this paediatric service the doctors had all been
educated in hospitals for children of the “Capital
Federal” and had an extensive experience in the
TIN :Terapia intensiva neonatal.

Con el fin de exponer el trabajo con padres ,
por un lado describiré la organizacion del “Equipo
de interconsulta psicológica “, que acudía las 24
horas del día durante los 7 días de la semana a
demanda del staff médico Y por el otro me refiriré
muy resumidamente a dos casos en donde los
niños fallecieron después de una larga internación,
incluyendo algunas efectos observados en
el equipo de salud.
Para ilustrar el trabjo en TIN leeré un resumen
sobre su organización y comentaré algunos resultados
de una encuesta que se le efectuó a los
padres.
Matizaré el relato con comentarios sobre los
modelos teóricos en pugna por aquellos años,
sus aportes y sus problemas en relación a la
clínica.

Implementation required a great eff ort from
everyone. Firstly, although we were working in a
highly specialized Neonatological and Paediatric
Intensive Care Unit in Latin America we couldn’t
count on an specific space so the interviews with
the parents were held in the corridors or bars, or at
the nursery with the parents of children admitted
in the neonatological intensive care unit, accommodating
all to the lack of resources.
There was no time to interact with the general
clinicians being the good will of all professionals
the only way to make communication possible.
