4 - CHILDREN´S TREATMENT OUTCOME IN A PARTIAL TIME CARE UNIT<br />

Introduction: Área de Dia is a partial time care<br />

unit created in October 2000 at the Department<br />

of Child and Adolescent Psychiatry of Hospital<br />

Dona Estefânia for children aged 6 to 12 with<br />

disruptive behaviour disorders. This unit provides<br />

long-term care (foreseen duration of treatment<br />

= two years) and multimodal intervention<br />

for the child, family and school.<br />

Objectives: The purpose of this study is to evaluate<br />

the evolution of the children who attend this<br />

unit. We aim to test the following hypothesis:<br />

children who attend Área de Dia show a favourable<br />

evolution at the end of he treatment.<br />

Methods: This is a prospective study that compares<br />

data collected at admission and at the end<br />

of the treatment. The study’s population consisted<br />

of children who attended and completed the<br />

second year of the treatment program, between<br />

2004 and 2008 (N=25).<br />

Treatment outcome was assessed from the<br />

parents´, teachers´ and clinicians´ perspectives.<br />

Multiple questionnaires and rating scales were<br />

used, including the Functioning Global Assessment<br />

Scale (FGA) and a Signs and Symptoms<br />

Checklist for the clinicians and the Strengths and<br />

Difficulties Questionnaire (SDQ- Por) for parents<br />

and teachers. Parental satisfaction and parental<br />

cooperation were also evaluated, using two<br />

questionnaires developed by the authors.<br />

Results: There is a significant positive change in<br />

the children’s behaviours both in the clinicians’<br />

reports (Signs and Symptoms Checklist and<br />

FGA) and in the Conduct Problems subscale reported<br />

by parents. No other change is reported<br />

by parents. Nevertheless, the results show a<br />

high degree of parental satisfaction and a high<br />

level of cooperation with this intervention.<br />

Results in teachers’ reported scores show a<br />

significant positive change in Hyperactivity. Even<br />

if there isn’t a statistical correlation between<br />

scores in the remaining of the questionnaire’s<br />

subscales, one can perceive a positive shift towards<br />

normality, except in the “Problems with<br />

Peers Relations” subscale. In this subscale the<br />

items are mainly behaviour that involves isolation<br />

or social restrictions, affecting socialization<br />

and peer relations.<br />

As a result the negative tendency for peer relations<br />

can result in behaviour such as social retraction,<br />

with a lesser ability to establish relationships,<br />

and this can be seen by adults as a sign<br />

that the clinical situation is worsening. However,<br />

these results appear to us to correspond to the<br />

emergence of the depressive aspects of children,<br />

initially masked by agitation and very aggressive<br />

behaviour. In this sense, they reflect a<br />

positive evolution, potentially preparing the children<br />

for a higher therapeutic level and making<br />

possible a new relational model.<br />

Conclusions: The results of the study partially<br />

confirm our hypothesis. Furthermore, we noticed<br />

that the major strength of the program seems to<br />

rely on the support provided to parents. We aim<br />

to continue this study in the following years of<br />

our treatment program.
