courses of action, justification and <strong>in</strong>dependent review by the utility. A root cause<br />

analysis has to be undertaken to ensure that the real cause of the problem (e.g. <strong>in</strong>cident) is<br />

identified. The regulator can check evidence of the adequacy through monitor<strong>in</strong>g the<br />

occurrence of similar problems.<br />

In all cases the emphasis must be on systematic and conservative assumptions that can be<br />

related to risk and the accepted safety criteria. The fundamental pr<strong>in</strong>ciples of safety<br />

culture namely, prudent and rigorous approach, question<strong>in</strong>g attitude, and communications<br />

underp<strong>in</strong> the need for all safety case submissions and root cause analyses to be carried out<br />

with due regard for the possible consequences.<br />

Whereas sufficient attention may be devoted to technical plant modifications, the same is<br />

not always true for changes <strong>in</strong> organizational systems. Yet it is the latter that may have<br />

very serious consequences for the ability of the organization to develop a sound safety<br />

culture.<br />

Lack and failure of <strong>in</strong>dependent nuclear safety reviews<br />

For all nuclear safety‐significant proposals and modifications, <strong>in</strong>dependent nuclear safety<br />

assessments should be undertaken by persons other than those who have undertaken the<br />

orig<strong>in</strong>al work. In a healthy safety culture, these assessments will always have been fully<br />

documented, and checked for methodological, calculation and technical accuracy and<br />

validity, us<strong>in</strong>g approved procedures.<br />

Violations<br />

Non‐compliance (violations) tends to be recorded by most licensees to vary<strong>in</strong>g degrees, <strong>in</strong><br />

relation to breaches of operat<strong>in</strong>g rules and operat<strong>in</strong>g <strong>in</strong>structions. Such reports can be of<br />

variable quality and detail. Not only do violations provide a rich <strong>in</strong>sight <strong>in</strong>to the<br />

operational performance and compliance characteristics of any organization, and a fertile<br />

ground for <strong>in</strong>vestigation <strong>in</strong>to general and specific problem areas, but they also serve to
