Operations documents are kept up to update, and technical details conta<strong>in</strong>ed <strong>in</strong> the<br />

procedures and reference material are consistent. A systematic approach with assigned<br />

responsibilities is provided to keep procedures and reference material up to date <strong>in</strong> a<br />

timely manner. The system allows for temporary procedure modifications, and provides<br />

for satisfactory distribution to ensure that new procedures reach all staff concerned, and<br />

previous outdated versions are replaced. There are adequate mechanisms <strong>in</strong> place for<br />

operat<strong>in</strong>g staff to report and document mistakes <strong>in</strong> operat<strong>in</strong>g procedures. Regular reviews<br />

are conducted to ensure that the documentation available <strong>in</strong> the ma<strong>in</strong> control room and <strong>in</strong><br />

the field is up to date, and that any temporary <strong>in</strong>formation displayed for the operations<br />

staff is current and properly authorized.<br />

15.2.5. Conduct of operation<br />

Operations personnel are controll<strong>in</strong>g over the plant systems and components <strong>in</strong> all modes<br />

of operation. The shift supervisor is <strong>in</strong>formed of plant activities affect<strong>in</strong>g the status of<br />

systems and components, and all activities such as carry<strong>in</strong>g out surveillance tests and<br />

ma<strong>in</strong>tenance works are routed via him or his delegate for f<strong>in</strong>al approval. Similarly, it is<br />

important to keep the operators <strong>in</strong>formed of plant status.<br />

A formal communication system exists for the transmission of orders and for the transfer<br />

of <strong>in</strong>formation related to the reliable and safe operation of the plant. As part of this<br />

system, plant modifications and major tests are properly authorized by management and<br />

controlled by operations personnel <strong>in</strong> accordance with the established procedures. This<br />

system <strong>in</strong>cludes retrievable record<strong>in</strong>g of orders and <strong>in</strong>structions, with affirmations that<br />

orders and <strong>in</strong>structions have been received and understood.<br />

It is important that close adherence to written procedures is observed <strong>in</strong> order to ensure<br />
