11. Human Performance: A Perspective on Human Factors<br />

Include other situational <strong>in</strong>formation that characterizes the work context:<br />

Characterization of environmental work conditions.<br />

Characterization of the man mach<strong>in</strong>e <strong>in</strong>terface and its limitations.<br />

Information on the tools used and their limitations, <strong>in</strong>clud<strong>in</strong>g available procedures<br />

and work documents.<br />

System/process characteristics, which are relevant for a good understand<strong>in</strong>g of the<br />

human error/behavior (e.g. degree of tolerance of system/process for human error,<br />

presence of a pitfall situation, etc.).<br />

Specific plant conditions which might have had direct <strong>in</strong>fluence on the operator’s<br />

performance, such as pre‐event plant configuration (<strong>in</strong>clud<strong>in</strong>g recent performance history<br />

of equipment), process parameters and other factors <strong>in</strong>clud<strong>in</strong>g off‐nom<strong>in</strong>al and/or<br />

dynamic conditions which resulted <strong>in</strong> unusual plant configurations and plant behavior.<br />

Figures, <strong>in</strong>clud<strong>in</strong>g layouts, photographs or draw<strong>in</strong>gs which allow illustration of the<br />

environment <strong>in</strong> which a failure occurred can help analysis of the man‐mach<strong>in</strong>e‐<strong>in</strong>terface<br />

and environmental factors.<br />

Other situational <strong>in</strong>formation that characterizes the organization of the work <strong>in</strong>cludes:<br />

Information on personnel work schedul<strong>in</strong>g (<strong>in</strong>clud<strong>in</strong>g tim<strong>in</strong>g of activities; e.g.,<br />

<strong>in</strong>dicate if the problem was related to night work; time pressure due to work on<br />

critical path, etc.);<br />

Information on the type of supervision and the supervisory methods;<br />

Organizational aspects <strong>in</strong>clud<strong>in</strong>g task assignment and task process<strong>in</strong>g (work<br />

plann<strong>in</strong>g and preparation), shift/team size and composition, and use of contractors;<br />

Analysis of if, and to what extent, on the day of the event, the context was different<br />
