11. Human Performance: A Perspective on Human Factors<br />

regulatory report<strong>in</strong>g requirements, legal issues, etc., as well as some reluctance to provide<br />

<strong>in</strong>formation related to human performance with<strong>in</strong> plants and outside. In this respect the<br />

establishment of a blame‐free report<strong>in</strong>g culture should be promoted. It is also recognized<br />

that a condition for improved human factor report<strong>in</strong>g is the availability and systematic<br />

use of human and organizational factors oriented event <strong>in</strong>vestigation methods.<br />

One of the identified potential problems <strong>in</strong> analyz<strong>in</strong>g, report<strong>in</strong>g and retriev<strong>in</strong>g human<br />

factor related <strong>in</strong>formation is the lack of use of a common language. It was agreed that a<br />

possible way to overcome this problem would be by reach<strong>in</strong>g a consensus on the<br />

taxonomy to be used regard<strong>in</strong>g report<strong>in</strong>g of human and organizational factors. The way<br />

chosen to <strong>in</strong>troduce such a common taxonomy was by provid<strong>in</strong>g, on the one hand, a<br />

limited list of human and organizational factor codes, rema<strong>in</strong><strong>in</strong>g manageable and<br />

preserv<strong>in</strong>g the odds on a consistent cod<strong>in</strong>g, and on the other hand, a longer list of<br />

keywords support<strong>in</strong>g each proposed code.<br />

11.3.2. The Ma<strong>in</strong> Steps to Human Factors Event Analysis<br />

This guidance may be used for those events where human performances were deficient or<br />

where organizational problems were highlighted.<br />

The first step: clarify what happened dur<strong>in</strong>g the event<br />

The description of human actions and related organizational aspects may enable the<br />

reader to have a clearer picture of human behaviour and of the technical and<br />

organizational environment <strong>in</strong> which the event occurred. The analysis beg<strong>in</strong>s by<br />

exam<strong>in</strong>ation of some chronological and situational <strong>in</strong>formation, and of a description of<br />

the recovery process.<br />
