11.3. THE EVENT ANALYSIS WITH AN HUMAN<br />

FACTORS APPROACH<br />

11.3.1. Background on Human Factors Event Analysis<br />

The efficiency of the operat<strong>in</strong>g experience feedback process, and the possibilities of<br />

retriev<strong>in</strong>g lessons learned, which is the ma<strong>in</strong> purpose of utilities, regulators and IRS, have<br />

been seriously hampered <strong>in</strong> the past by observed deficiencies <strong>in</strong> the quality, consistency<br />

and completeness of <strong>in</strong>formation <strong>in</strong> the identification of human factors aspects of events<br />

related to human performance issues.<br />

When the NEA and IAEA report<strong>in</strong>g systems were merged together <strong>in</strong> 1997, new<br />

guidel<strong>in</strong>es for report<strong>in</strong>g were developed. In order to strengthen the report<strong>in</strong>g of events<br />

<strong>in</strong>volv<strong>in</strong>g human factors, new recommendations and cod<strong>in</strong>g system were developed <strong>in</strong> co<br />

‐operation with WANO (World Association of Nuclear Operators). Their purpose is to<br />

identify the type and detail of <strong>in</strong>formation on human and organizational factors that<br />

should be expected and to provide guidance on how this <strong>in</strong>formation could be researched<br />

and presented with<strong>in</strong> the body of the event reports.<br />

In order to respect this time frame, a pragmatic approach had to be followed, mak<strong>in</strong>g<br />

maximal use of exist<strong>in</strong>g documents. The guidance tried to be not too difficult to use by<br />

those who are not human factor specialists and who do not necessarily have specific<br />

tra<strong>in</strong><strong>in</strong>g <strong>in</strong> human factor analysis techniques.<br />

In this area, <strong>in</strong>herent limitations of human factor analysis are due to national culture,<br />
