from a normal (<strong>in</strong>cident‐free) activity performance.<br />

Thirdly, <strong>in</strong>formation on the nature and tim<strong>in</strong>g of recovery actions may provide additional<br />

<strong>in</strong>sights <strong>in</strong>to the complexity of the situation and the difficulties for the operators to detect<br />

and diagnose the problems at hand. Also, lessons may be learned from the positive roles<br />

of the NPP personnel <strong>in</strong>volved <strong>in</strong> an event, <strong>in</strong>clud<strong>in</strong>g:<br />

How and when the recovery was achieved (recovery <strong>in</strong> immediate response to the<br />

error, recovery by planned action or by trouble shoot<strong>in</strong>g, recovery directed or not<br />

directed by provided procedures);<br />

Who was <strong>in</strong>volved <strong>in</strong> the recovery actions (recovery performed by the person who<br />

committed the error himself, or by the team to which this person belongs, or by<br />

completely different persons or group);<br />

What is the time delay needed for the detection of the human error or system<br />

failure (<strong>in</strong>clud<strong>in</strong>g latent failures) and for the diagnosis of the safety<br />

problem/degradation? Expla<strong>in</strong> the reasons for long time delays before the problem<br />

was detected or diagnosed, if applicable.<br />

The second step: demonstrate why is the event safety significant?<br />

The analysis is cont<strong>in</strong>ued by the safety analysis based on this <strong>in</strong>formation. The safety<br />

significance of the actual and potential consequences of the observed human performance<br />

problems are discussed, <strong>in</strong>clud<strong>in</strong>g consideration of potential consequences on the<br />

operability of the plant systems and on the subsequent ability of plant staff to handle plant<br />

components.<br />

This part may <strong>in</strong>clude a discussion of the barriers that were broken by the identified<br />

human performance deficiencies (loss of defense <strong>in</strong> depth) and the effectiveness of the<br />
