If the regulatory body makes a request, which, <strong>in</strong> the op<strong>in</strong>ion of the operat<strong>in</strong>g<br />

organization could have an adverse effect on safety, then, <strong>in</strong> view of prime responsibility<br />

for safety, this op<strong>in</strong>ion should be presented to the regulatory body as a basis for further<br />

discussions.<br />

15.1.1.3. Operational experience feedback<br />

Operat<strong>in</strong>g experience at the plant shall be evaluated <strong>in</strong> a systematic way. Similarly, the<br />

operat<strong>in</strong>g organization shall obta<strong>in</strong> and evaluate <strong>in</strong>formation from operational experience<br />

at other plants that provides lessons for the operation of its own plant. To this end, the<br />

exchange of experience and the contribution to national and <strong>in</strong>ternational organizations is<br />

of great importance.<br />

Abnormal events with important safety implications shall be <strong>in</strong>vestigated to establish<br />

their direct and root causes. The <strong>in</strong>vestigation shall result <strong>in</strong> clear recommendations to<br />

plant management who shall take appropriate corrective action. Information shall be fed<br />

back to the plant personnel. All plant personnel shall be required to report all events and<br />

encouraged to report near misses relevant to the safety of the plant.<br />

Plant management shall ma<strong>in</strong>ta<strong>in</strong> liaison as appropriate with the organizations <strong>in</strong>volved <strong>in</strong><br />

the design (manufacturer, research organization, designer), with the aims of feed<strong>in</strong>g back<br />

operat<strong>in</strong>g experience and of obta<strong>in</strong><strong>in</strong>g, if needed, valuable advice <strong>in</strong> case of equipment<br />

failures or abnormal events.<br />

Data from operat<strong>in</strong>g experience shall be collected and reta<strong>in</strong>ed for use as <strong>in</strong>put to plant<br />

age<strong>in</strong>g management, residual life evaluation, probabilistic safety assessment and periodic<br />

safety review.<br />

15.1.1.4. Fire safety<br />

The operat<strong>in</strong>g organization shall have fire safety arrangements, based on a periodically<br />
