implemented. Prior to its execution, it can be assessed accord<strong>in</strong>g to whether or not it has<br />

sufficient cont<strong>in</strong>gencies, displays soundness of judgment, imag<strong>in</strong>ation, flexibility,<br />

awareness of detail and the like.<br />

There is no universally agreed classification of human error, nor is there one <strong>in</strong> prospect.<br />

A taxonomy is usually made for a specific purpose, and no s<strong>in</strong>gle scheme is likely to<br />

satisfy all needs. Nearly everyone who has published <strong>in</strong> this field has devised some form<br />

of error classification. Consequently, the literature abounds with such taxonomies,<br />

reflect<strong>in</strong>g a variety of practical concerns and theoretical orientations and rang<strong>in</strong>g from the<br />

highly task specific to broad statements of underly<strong>in</strong>g error tendencies.<br />

A. Model of Violation<br />

Figure 11.1 shows the graphical model of violation. Although the person has a good<br />

understand<strong>in</strong>g of the “prescribed task”, he/she develops an <strong>in</strong>tention not <strong>in</strong> compliance<br />

with his/her understand<strong>in</strong>g. The reason for this may be derived from different sources<br />

(<strong>in</strong>ternal or external). This k<strong>in</strong>d of behavior is called a violation. As an example, the<br />

operator wants to follow his “personal <strong>in</strong>tention” due to personal motivation (e.g., the<br />

operator does not follow the procedure <strong>in</strong> order to go home earlier etc.). In this case the<br />

operator is fully responsible for his/her action.<br />

There are other cases, <strong>in</strong> which the part of responsibility must be <strong>in</strong>vestigated <strong>in</strong> more<br />

detail and more general situations must be taken <strong>in</strong>to account. For <strong>in</strong>stance, (a) the<br />

person wants to satisfy other criteria like personal safety aspects, power production or<br />

even the efficiency of the larger project that <strong>in</strong>cludes this particular task.<br />

Recommendations have to be addressed to this k<strong>in</strong>d of compromise, someth<strong>in</strong>g must be<br />

done to clarify the decision mak<strong>in</strong>g; (b) the person th<strong>in</strong>ks that he knows a better solution<br />

than provided by the procedure; (c) the person can't perform the task because the<br />
