Repeated requests for dispensation to regulatory requirements<br />

Requests for dispensations to exist<strong>in</strong>g regulatory requirements can occur, particularly<br />

prior to restart after a planned outage. When requests are frequent this should trigger a<br />

review of the adequacy of the regulatory requirement, or of whether production priorities<br />

are be<strong>in</strong>g overemphasized at a possible disadvantage to safety<br />

Excessive hours of work<br />

A significant factor <strong>in</strong> the degradation of personal performance is fatigue. Safety culture<br />

relies on optimum output <strong>in</strong> the areas of attention, question<strong>in</strong>g attitude, diligence and<br />

fitness for duty, however, all these are adversely affected when a person is tired and<br />

stressed. Work<strong>in</strong>g hours must be formulated and regulated to allow <strong>in</strong>dividuals to perform<br />

their allotted duties with<strong>in</strong> reasonable time scales, without impos<strong>in</strong>g undue pressures<br />

which can <strong>in</strong>duce unsafe and undesirable consequences.<br />

Number of persons not complet<strong>in</strong>g adequate tra<strong>in</strong><strong>in</strong>g<br />

<strong>Tra<strong>in</strong><strong>in</strong>g</strong> plays an <strong>in</strong>tegral role <strong>in</strong> the safety culture of an organization and the regulator<br />

would want to be assured that adequate attention was be<strong>in</strong>g paid to the quality and<br />

applicability of tra<strong>in</strong><strong>in</strong>g programmes. These aspects are checked by submission,<br />

exam<strong>in</strong>ation and acceptance of the tra<strong>in</strong><strong>in</strong>g required by the regulator, however, the<br />

attendance and performance of staff at tra<strong>in</strong><strong>in</strong>g sessions needs ongo<strong>in</strong>g attention. When<br />

this <strong>in</strong>formation is correlated with the results of occurrence analysis, particularly if<br />

groups or departments are highlighted, it provides support<strong>in</strong>g evidence that further<br />

<strong>in</strong>vestigation and targeted corrective action may be needed <strong>in</strong> the area of tra<strong>in</strong><strong>in</strong>g.<br />
