21. Safety Culture<br />

All nuclear plant operations should employ suitably qualified and experienced persons.<br />

Whilst this is a basic requirement, and even a licence condition for many operat<strong>in</strong>g<br />

regimes, it is apparent that it cannot always be achieved <strong>in</strong> practice. Such failure tends to<br />

show itself <strong>in</strong> those <strong>in</strong>cident and accident event reports that conclude that further<br />

tra<strong>in</strong><strong>in</strong>g/retra<strong>in</strong><strong>in</strong>g etc. of personnel is required. Suitably qualified and experienced<br />

persons can be readily identified and recruited, however, by careful attention to the needs<br />

of a given job. This proactive approach <strong>in</strong>cludes identification of the pr<strong>in</strong>cipal duties and<br />

responsibilities of the job holder, the attributes required for the tasks to be performed, and<br />

the preparation of a profile outl<strong>in</strong><strong>in</strong>g the characteristics that would be required of any job<br />

<strong>in</strong>cumbent <strong>in</strong> order to carry out the duties effectively. The presence of unsuitable and<br />

<strong>in</strong>experienced personnel becomes readily apparent when checks are made regard<strong>in</strong>g<br />

knowledge and experience requirements aga<strong>in</strong>st the basic skill, knowledge, and task<br />

capability that is available with<strong>in</strong> a workforce. Such checks can be<br />

made by means of skills and job task analysis.<br />

Understand<strong>in</strong>g of job descriptions<br />

Typically <strong>in</strong> poor safety cultures, some <strong>in</strong>dividuals are not fully aware of the full<br />

requirements, responsibilities and accountabilities of their job. This can arise either<br />

because job descriptions have not been properly prepared <strong>in</strong> the first <strong>in</strong>stance, or else<br />

because <strong>in</strong>dividuals have not been properly briefed on the expectations of their employer.<br />

In either case, it will be apparent that there is the potential for a significant mismatch<br />

between the expectations of the employer and the employee. In order to check that this is<br />

not a safety culture concern, the licensee should produce the necessary safety components<br />

of the relevant job descriptions.<br />

Contractors<br />

Trend<strong>in</strong>g and analysis of occurrences or problems may reveal contractor <strong>in</strong>volvement and<br />

shortcom<strong>in</strong>gs. Exam<strong>in</strong>ation of the contract specifications and conditions prior to contract
