15. Operational Safety<br />

− The number of alarms that are <strong>in</strong> an activated condition dur<strong>in</strong>g normal<br />

operation is m<strong>in</strong>imized.<br />

− The number of alarms aris<strong>in</strong>g per hour dur<strong>in</strong>g start‐up of the plant is m<strong>in</strong>imized<br />

and does not exceed the operator's ability to respond.<br />

− Defective or out of calibration <strong>in</strong>strumentation is clearly <strong>in</strong>dicated to the<br />

operator and tagged.<br />

Arrangements and procedures for logg<strong>in</strong>g data, <strong>in</strong>k<strong>in</strong>g and dat<strong>in</strong>g recorders, etc.,<br />

are correctly implemented for effective identification, distribution, and storage of<br />

records.<br />

Shift turnover is carried out <strong>in</strong> a structured and professional manner. The effectiveness of<br />

shift turnover is enhanced by a written account of shift activities. In order to achieve<br />

common agreement and set objectives, a shift brief<strong>in</strong>g is conducted dur<strong>in</strong>g turnover. The<br />

turnover from one shift to the next gives particular attention to the transfer of all the<br />

important <strong>in</strong>formation about the plant status, work <strong>in</strong> progress, and events dur<strong>in</strong>g the<br />

shift. Checklists, log books, record<strong>in</strong>gs and messages are used and the operators perform<br />

a walk down of the control panels.<br />

Regular rounds through the plant are performed by the operators. Also shift supervisors<br />

and authorized staff adopt the practice of regular plant walk‐throughs. All relevant areas<br />

are covered with<strong>in</strong> specified <strong>in</strong>tervals. Areas with higher radiation levels have to be<br />

observed at less frequent <strong>in</strong>tervals or by remote means. The scope of the rounds covers<br />

leakage, excessive vibration, unfamiliar noise, foreign parts, and deficiencies requir<strong>in</strong>g<br />

ma<strong>in</strong>tenance, clean<strong>in</strong>g, or other action. Special arrangements are necessary for areas that<br />

cannot be entered dur<strong>in</strong>g power operation. A system for document<strong>in</strong>g problems exists<br />

which <strong>in</strong>cludes an evaluation for operability impact.
