15. Operational Safety<br />

The OEF program should be effective. Effectiveness appears through root cause<br />

<strong>in</strong>vestigations, process<strong>in</strong>g of an external OEF experience, OEF <strong>in</strong>formation meet<strong>in</strong>gs, and<br />

through tra<strong>in</strong><strong>in</strong>g sessions <strong>in</strong>volv<strong>in</strong>g OEF <strong>in</strong>formation. Important issues are number of<br />

significant events, repeated events, events that appear similar to known external<br />

experiences, and events that are shared with the external nuclear community.<br />

Procedures are provided for the OEF program, <strong>in</strong>clud<strong>in</strong>g: a description of the OEF<br />

program, clearly def<strong>in</strong>ed responsibilities, precise guidance on how to process OEF<br />

<strong>in</strong>formation, and some means of status report<strong>in</strong>g. This activity is preferably co‐ord<strong>in</strong>ated<br />

by one <strong>in</strong>dividual, and specific guidance on it is given <strong>in</strong> written procedures.<br />

The OEF procedures have a systematic method for document<strong>in</strong>g various <strong>in</strong>‐house events<br />

and the type of events be<strong>in</strong>g documented <strong>in</strong>clude both significant and non‐consequential<br />

events. The shift operat<strong>in</strong>g logs for events are considered for <strong>in</strong>vestigation. The screen<strong>in</strong>g<br />

process of <strong>in</strong>‐house events <strong>in</strong>cludes precursor events. The root‐cause <strong>in</strong>vestigation process<br />

is carried out by tra<strong>in</strong>ed staff members experienced with various root‐cause techniques.<br />

The safety performance of the plant is monitored through a process <strong>in</strong>clud<strong>in</strong>g specific<br />

safety performance <strong>in</strong>dicators, such as, the number and severity of significant events, the<br />

number of outstand<strong>in</strong>g work orders, and the duration of unavailability of safety systems.<br />

They are monitored, trended and analyzed by a group with<strong>in</strong> the Technical Support Section<br />

of the plant. Safety performance <strong>in</strong>dicators are rout<strong>in</strong>ely reported to and reviewed by the<br />

plant safety review group or Nuclear Safety Committee. The plant manager, department<br />

managers and corporate management are rout<strong>in</strong>ely updated on the safety performance
