unless the operat<strong>in</strong>g organization has a very high level of commitment to safety. The lead<br />

<strong>in</strong> safety matters must come from the top, from the highest levels of management. Their<br />

safety policies and attitudes need to permeate the operat<strong>in</strong>g organization on every level<br />

and to extend to other organizations perform<strong>in</strong>g delegated tasks. There can be no<br />

complacency at any level about the cont<strong>in</strong>uous attention demanded by safety. Safety<br />

management implies a learn<strong>in</strong>g attitude to safety matters and the open exchange of<br />

<strong>in</strong>formation both upwards and downwards <strong>in</strong> the organization.<br />

The operat<strong>in</strong>g organization will usually delegate operat<strong>in</strong>g authority to the on‐site<br />

management of the <strong>in</strong>stallation that has the direct day‐to‐day control. Accord<strong>in</strong>gly, the<br />

operat<strong>in</strong>g organization has a responsibility to monitor the effectiveness of safety<br />

management at the <strong>in</strong>stallation and to take necessary measures to ensure that safety is<br />

ma<strong>in</strong>ta<strong>in</strong>ed at the desired level.<br />

3.2.3.2. Quality assurance<br />

Quality assurance practices are an essential part of good management and are to be<br />

applied to all activities affect<strong>in</strong>g the quality of items, processes and services important to<br />

safety. Inherent <strong>in</strong> the achievement of quality is the adoption of a quality assurance<br />

programme, which <strong>in</strong>cludes the planned and systematic actions necessary to provide<br />

adequate confidence that specified requirements are satisfied. Implementation of the<br />

quality assurance programme <strong>in</strong>volves managers, performers of tasks, and those<br />

responsible for verification and assessment of the effectiveness of the programme. It is<br />

not the sole doma<strong>in</strong> of a s<strong>in</strong>gle group. However, management has the key responsibility<br />

to ensure that the programme functions properly and to establish and cultivate pr<strong>in</strong>ciples<br />

that <strong>in</strong>tegrate quality assurance practices with daily work activities.<br />

Quality needs to be verified by a discipl<strong>in</strong>ed approach. Thus, quality assurance practices<br />

<strong>in</strong>clude:<br />
