CHAPITRE I-1Mortalité : incidence, causes et facteurs de risqueird-00718213, version 1 - 16 Jul 2012Le petit nombre de patients coinfectés (dix) recommandeune interprétation prudente de cette association.Une étude faite en Gambie avant l’ère des TARVne retrouvait pas de différence de mortalité entre lesPVVIH coinfectés et ceux infectés par le VIH-1 (50).Notre cohorte de PVVIH présentait des caractéristiquesinitiales similaires à celles de la plupart descohortes suivies dans les PRML mais avec unedurée de suivi supérieure. Bien que les moléculesARV et les critères de mise sous TARV aientchangé, les PVVIH sous TARV dans ces pays ont euà être exposés aux mêmes molécules dont le d4T etles IP non boostés. L’exploitation des CDD s’estfaite à l’aide des techniques les plus utilisées dansles PRML. Il a été cependant récemment développéune classification des CDD (51) qui pourrait aider àune meilleure comparaison des CDD. Une reclassificationbasée sur celle-ci pourrait permettre unemeilleure confrontation de nos résultats avec ceuxdes pays riches.Les résultats de notre étude pourraient être généralisablesaux pays d’Afrique subsaharienne où le TARVa été disponible au début des années 2000. Ils soulèventune problématique peu explorée dans les paysdu Sud et identifient des facteurs qui contribuent àune moindre efficacité à long terme du TARV. Leurprise en compte dans les modèles de suivi des PVVIHdans les pays du Sud devrait contribuer à améliorer lasurvie après plusieurs années sous TARV. Les stratégiessuivantes peuvent être envisagées pour ce faire :– renforcement du dépistage de l’infection à VIH pourune initiation plus précoce du TARV,– amélioration de la prise en charge nutritionnelle desPVVIH,– meilleure gestion des échecs thérapeutiques : miseà disposition d’outils de monitorage adéquats (chargevirale) et des traitements de 2e ligne et de 3e ligne,– amélioration des stratégies diagnostiques etthérapeutiques des affections opportunistes, prioritairementla tuberculose et les affections cérébroméningées,– intégration du dépistage et du traitement descomorbidités non infectieusesPlus particulièrement pour le Sénégal, le dépistage àl’initiative du prestataire, la disponibilité de nouvellesclasses de TARV, des mesures de prévention ou decontrôle de la maladie cardiovasculaire, des hépatitesvirales, du cancer du col de l’utérus semblentpertinents pour améliorer la survie à long terme desPVVIH sous TARV.
