AtUrethritis. 8this stage the visible portion of the urethral mucons membrane isalready redder and swollen. The peculiar titillation is followedan increased desire to urinate and increased erections, sometimesalso by nocturnal emissions' In a few days the titillation changeto a pain, the urging to urinate becomes more frequent, the painurinating becomes intolerable, and the urine of which only a fewdrops are emitted, becomes scalding hot, so that it seems like fiIf the urine is emitted in larger quantity, the pains during theemission are less severe, but so much more violent immediatelyafter. The concentrated urine, moreover, causes an increased painon which account the first discharge of urine early in the morninis the most distressing. The orifice of the urethra now begins toswell quite perceptibly, the lips look very red. The secretion stremains scanty, looks yellow or greenish-yellow, is thick, stiffethe linen with yellow or greenish-yellow stains, and, in the mornis easily squeezed out in an increased quantity. The sensitivenesof the urethra to pressure now extends to the fossa navicularis. The erections, especially at night, become more frequent and continuous, causing the patient great pain in consequence of the string of the exceedingly sensitive urethra; emissions now occur verrarely. Shortly after this, the purulent secretion increases in qtity, but preserves its greenish-yellow color. The parts stainedthe discharge, the glans and prepuce, swell ; the smegma is secremore abundantly, excoriations and small superficial ulcers showthemselves. The inflammatory irritation of the prepuce easily results in phimosis or paraphimosis. The further extension of thedisease is evident from the fact that the urethra now is sensitivits whole length. It is only in a few cases that the gonorrhoearemains confined to the anterior portion of the urethra. This socalled inflammatory stage seldom lasts less than eight days, geneally a fortnight. General febrile symptoms are seldom present;what might be considered as such, is owing to the mental excitement of the patient and to the intensity of the pain. Towards theend of this first stage the discharge generally becomes very copibut its color changes more and more to that of a whitish mucus, and assumes a more fluid consistence. At the same time the painsat urinating abate almost entirely, leaving at most only a disagrable feeling, the erections become less frequent and painful ; onthe other hand, the nocturnal emissions are apt to be more frequeand to be followed by slight exacerbations. The white secretionleaves gray and stiff stains on the linen, with a yellowish point4 Diseases of the Male Sexual Organs.the centre.
