Generally inflammations of the heart are consecutive or secondary818 Diseases of the Heart.affections arising either by a continuation of the inflammatoryprocess from the lungs and pleura to the heart ; or in consequencof the heart becoming involved in affections of more remote partsespecially of the kidneys, and likewise of the stomach and spleenor else, they may result from the operation of constitutional diseases. Among the latter acute rheumatism occupies the first rank, for there is not a case of intensely-acute articular rheumatism tis not, at the same time, accompanied by an inflammatory aftectioof the heart. We mention, moreover, scarlatina, variola, measles, typhus, and two other affections which are so apt to occasion aninflammation in every serous membrane, we mean pyaemia andpuerperal fever. As regards chronic constitutional affections, itcannot be shown that they act as exciting causes in inflammationsof the heart. Like all other inflammatory aflections, inflanmiartions of the heart sometimes occur in such numbers that we feeldisposed to trace their origin to telluric or atmospheric influenEtiologically the various forms of cardiac inflammation are distinguished so far as this, that myocarditis occurs least frequentas a primary disease, but generally develops itself out of one ofother forms, both of which occupy the same rank in all otherrespects. An exact knowledge of the anatomical changes occasioned bycardiac inflammation is not only essential to an intelligent apprciation of the single symptoms, but likewise to a knowledge of thpossible consequences of such inflammations. We can here onlyindicate the main points.Pericarditis has all the characteristics of an inflammation ofother serous membranes. It is very seldom diffuse and is mostcommonly limited to single spots. The pericardium at first looksinjected, opaque, sometimes spotted in consequence of slight extrvasations, it is loosely-adherent and easily torn ; at a later pethe pericardium appears covered with coagulable lymph, whichmay be disposed as a delicately-organized membrane, or may havethe appearance of granulated fragments of exudation. An effusioninto the cavity of the pericardium is scarcely ever entirely absebut may vary considerably both in quantity and quality. It maybe from a few ounces to several pints. The exudation either consiof plastic lymph, or of plastic lymph and serum, and is not unfrequently mixed with blood, or hemorrhagic ; it very seldom consistof nothing but serum. The exuded fluid may undergo the following essential changes : A complete absorption may take place andCarditis. 849http://www.archive.org/stream/sciencetherapeu00kafkgoog/sciencetherapeu00kafkgoog_djvu.txt
