Cancer <strong>of</strong> the mammae is most commonly <strong>of</strong> a primarynature, hence the beginning <strong>of</strong> the cancerous disease.<strong>The</strong> causes <strong>of</strong> this disease can scarcely ever be determined withcertainty. If a blow or a contusion is generally regarded as thecause <strong>of</strong> this disease, it is probably because no better cause canfound, not because the disease has ever been known to result immediately from such an injury. <strong>The</strong> influence <strong>of</strong> depressing emotions which are so easily und so commonly succeeded by cancerousdegenerations, is a much more evident source <strong>of</strong> the cancerousCarcinoma Mammae. 101disease. Cancer <strong>of</strong> the rnammse most commonly occurs betweenthe ages <strong>of</strong> 40 and 50, about the period which is generally designated as a change <strong>of</strong> life. Unmarried women or women whohave not had children seem to be more liable to it than marriedwomen with children. Before and after this period, it is proportionally a rare occurrence.<strong>The</strong> most frequently observed form <strong>of</strong> carcinoma <strong>of</strong> the mammaeis scirrhus, next to w^hich we range the encephaloid or medullaryfungus, the alveolar or colloid cancer and the epithelial cancerepithelioma.[Mr. Maurice H. CoUis <strong>of</strong> Dublin, in his work "<strong>The</strong> Diagnosisand Treatment <strong>of</strong> Cancer, and the Tumors analogous to it" 1864,has thrown out epithelioma and colloid growths from the truecancer-group. He regards Colloid as a mere variety <strong>of</strong> fibroid orrecurrent tumors. Virchow says that " its stroma differs in its ctaining mucus and in its gelatinous nature from the ordinarystroma <strong>of</strong> cancer," page 526, On Tumors. Mr. Collis says, in rejecting epithelioma from the group <strong>of</strong> cancerous tumors: "Its superficial origin, its slow progress, its indisposition to infiltrdeeper structures, or to contaminate the glands, the certainty <strong>of</strong>cure which follows its timely removal, and the difterent appearanwhen occupying similar localities, are <strong>of</strong> suflicient importance toutweigh the points <strong>of</strong> resemblance which it undoubtedly bears tocancer in its advanced and secondary stages. In its early stage iis strictly an hypertrophy, and in this condition it may remain fan indefinite period. Its second stage is one <strong>of</strong> hypertrophy andulceration combined. This stage also, as far as external or cutaneous epithelioma is concerned, is slow to advance into the thirdor destructive stage, that <strong>of</strong> infiltration and secondary deposit,page 226. Speaking <strong>of</strong> Colloid, Virchow writes: "It remains for avery long time local, so that the nearest lymphatic glands <strong>of</strong>tennot become affected until after the lapse <strong>of</strong> years, and then agaithe process is for a long time confined to the disease <strong>of</strong> thelymphatic glands, so that a general outbreak <strong>of</strong> the disease in alparts <strong>of</strong> the body does not take place until late, and only in rarinstances."
