Sometimes the patients complain <strong>of</strong> a little hackingcough, and exhibit symptoms <strong>of</strong> a slight bronchial catarrh. <strong>The</strong>urine is less in quantity, saturated, notwithstanding that thepatients <strong>of</strong>ten experience a very violent thirst. All these symptoremain in force only in very violent cases; most generally a sensible remission <strong>of</strong> these symptoms takes place towards the end <strong>of</strong>the first week.In the second week the symptoms <strong>of</strong> the disease undergo amarked change, unless the morbid process takes a turn towardsrecovery, which is very seldom the case. For the present we onlyspeak <strong>of</strong> typhus <strong>of</strong> a moderately severe type. In this stage thepatients lose their consciousness more and more; they are eitherlying in a state <strong>of</strong> lethargic apathy, or else, it is only with grdifliculty that they are able to reply to questions or to give utance to their own ideas. In the evening and during the night thissoporous prostration is generally interrupted by a state <strong>of</strong> nervoexaltation, during which the patients manifest their internal nerousness by animated talking or by a constant endeavor to escape680 Epidemic and Endemic Infectious Diseases.from their beds. Sensations <strong>of</strong> pain now cease entirely; whenasked how they feel, they answer : " Quite well ; " they expressdesire for drink, but they swallow the <strong>of</strong>fered beverage hurriedlyand greedily ; when repeatedly asked to do so, they put out theirtongues slowly and tremulously, and forget to draw them in again.Another evidence that the influence <strong>of</strong> the brain is almost entiresuspended is, that the patients persevere for a long time in an ucomfortable position, and that they allow the urine and feeees toescape into their beds. At the beginning <strong>of</strong> the second, and veryfrequently already at the end <strong>of</strong> the first week, the patients complain <strong>of</strong> a violent buzzing in the ears, afterwards they are evidehard <strong>of</strong> hearing. Corresponding with these changes the countenances <strong>of</strong> the patients become altered. Although the complexionseems to shine, yet it has a livid hue; the eyes stare, or they ha vague and unsteady expression ; when raised in their beds, thepatients at once turn pale and look as if they would faint. <strong>The</strong>various functions show the following deviations from their normalcondition : <strong>The</strong> fever is intense, the temperature rises in the eving to 100° or 102° Fahr., with slight morning-remissions; thepulse is seldom below 100, nor is it <strong>of</strong>ten $tbove 120, it is weakthan usual and sometimes dicrotic. <strong>The</strong> tongue, which alreadyshowed a good deal <strong>of</strong> dryness in the first week, now is constantldry; the streaked coating, which had marked it hitherto, nowvanishes ; it shows a peculiarly red color, and the papillae seemhave become effaced ; towards the end <strong>of</strong> the second week thetongue appears covered with a brownish incrustation. <strong>The</strong> appetite is entirely wanting, yet the patients will taste <strong>of</strong> what the<strong>of</strong>fered to eat. <strong>The</strong>y do not seem thirsty, yet they drink greedilythe pr<strong>of</strong>fered beverage. At this stage the abdomen begins to bloatquite considerably ; when the ilio-coecal region is pressed uponhard, the patients distort the corners <strong>of</strong> their mouths.
