Cancer <strong>of</strong> the uterns is<strong>of</strong> the most frequently occurring fomis <strong>of</strong> carcinoma, and not unfrquently runs its course as a solitary cancer. It is only in rarethat it breaks out in addition to cancerous degenerations <strong>of</strong> otheorgans.<strong>The</strong> most ordinary form is medullary cancer, whereas scirrhusand alveolar cancer are very rare; epithelial cancer is a littlefrequent. <strong>The</strong> medullary cancer usually commences at the vaginalportion <strong>of</strong> the uterus as a very firm, rugged infiltration, whencespreads to the fundus and to adjoining organs, especially the blader and rectum. <strong>The</strong> less firmness it possesses, the sooner it becconverted into pus or ichor^ in consequence <strong>of</strong> which enormous destructions <strong>of</strong> tissue may take place. Epithelial cancer always commences with cauliflower-excrescences at the os tincfie so that thexcrescences seem to have taken the place <strong>of</strong> the os, or at any racoalesce with it. <strong>The</strong> excrescences seem to grow out o£ the papill<strong>of</strong> the OS ; at first they resemble condylomata and afterwards sprlike a fungus, with marked vascular development. As the fungoidgrowth increases, the elements <strong>of</strong> epithelial cancer unite with thformer and the mass decays and changes to ichor. It is not certaiwhether these excrescences are really carcinomatous at the outsetat any rate^ the fact, that they have been successfully extirpatethrows doubt on the correctness <strong>of</strong> the canceivhypothesis.Hartmann relates a form <strong>of</strong> disease the carcinomatous nature <strong>of</strong>which is doubtful, we mean the phagedssnic ulcer <strong>of</strong> the uterus.It likewise emanates irom the vaginal portion, but the ulcerativeprocess is not preceded by the deposition <strong>of</strong> a firm, pseudolayet* or infiltration ; the uterine tissue surrounding the ulceris sound, or else s<strong>of</strong>tened, yellow or reddish-brown* <strong>The</strong> disorgantion may gradually spread to the walls <strong>of</strong> the uterus and vagina,tiO the rectum, bladder and peritoneumiCarcinoma <strong>of</strong> the uterus is frequently mistaken at the commencetnent for menstrual disorder, leucorrhoea or chronic metritis. Thfirst symptoms usually are menstrual derangements, cessation ora more frequent return <strong>of</strong> the menses, or in the place <strong>of</strong> the menswe have irregular hemorrhages and leucorrhoea. <strong>The</strong> mistake ismade the more easily if the patient has not yet reached the critiperiod, so that these changes may be set down as results <strong>of</strong> a physiological cessation <strong>of</strong> the catamenial secretion.
