As long as the mitral valve remains intact, such ahypertrophy is characterized by the same phenomena as thosedescribed above, only in a more intense degree, and with shorter,or without any intermissions. But if, owing to the excessive dilatation, the mitral valve is no longer sufficient to close the aurventricular orifice, the consequence is that the more forcible exsion <strong>of</strong> the blood into the aorta becomes associated with a stasisthe lesser circulation and more immediately in the function <strong>of</strong> thright ventricle, in consequence <strong>of</strong> which the whole heart becomeshypertrophied in the most dangerous acceptation <strong>of</strong> this term. <strong>The</strong>impulse <strong>of</strong> the heart is perceived over a large extent by the eye,ear and touch ; it is even heard without the ear being applied tothe chest; the concussion <strong>of</strong> the thorax is extreme; the dulnessextends further on to the left, and is returned over a larger arethe heart descends below its normal line, the ribs even bulge. Thsounds <strong>of</strong> the heart are either mufiled, or else ringing, and, ifvalves are diseased, murmurs are heard. Other symptoms can easilybe inferred by any one who considers that the arteries are extremely engorged, and that the lungs are in a state <strong>of</strong> hyperoemiaAll these conditions will be dwelt upon more fully when we cometo treat <strong>of</strong> valvular anomalies.<strong>The</strong> phenomena emanating from the hypertrophied right ventricle, are very frequently entirely disguised by those <strong>of</strong> the aftion which had given rise to the hypertrophy. Anxiety, dyspnoea,oppression, palpitation <strong>of</strong> the heart, may be caused by pulmonaryaffections alone. Simple hypertrophy merely results in a greateraccumulation <strong>of</strong> blood in the lungs, and a consequent catarrhalaffection <strong>of</strong> the lungs, with hemorrhage, hypersemia <strong>of</strong> the lungs.<strong>The</strong> physical signs are : Increased extent <strong>of</strong> dulness on the rightside, even beyond the right border <strong>of</strong> the sternum; pulsation inthe pit <strong>of</strong> the stomach, perceptible raising <strong>of</strong> the lower border othe sternum; accentuation <strong>of</strong> the second sound <strong>of</strong> the pulmonaryartery.Hypertrophy with dilatation may be very considerable without8ui)erinducing any serious derangements. It implies a sort <strong>of</strong> uniform compensation <strong>of</strong> functional power, and the patients only per876 Diseases <strong>of</strong> the Heart.ceive a dirturbance <strong>of</strong> equilibrium if they undertake to undergosevere bodily exertions.
