Insuch cases an apparently trivial cause, such as a cold, a physicaexertion, a sudden outburst <strong>of</strong> passion, etc., is suflScient to girise suddenly to a very general oedema <strong>of</strong> the lungs <strong>of</strong> which deatmay be the speedy result.Acute oedema most frequently sets in during the death-strugglein the most diversified diseases.Symptoms* <strong>The</strong> first important and striking symptom is thedyspnoea; it may attain a very high degree, is most commonlyassociated with the sensation <strong>of</strong> an oppressive weight under thesternum which does not abate by any change <strong>of</strong> position ; at thesame time the expression in the countenance is one <strong>of</strong> anxiety, anthe 7njobility <strong>of</strong> the thorax is lesSy in consequence <strong>of</strong> which theat times sit up erect, at other times rest on their arms and agaistretch their heads forward, in order to facilitate the breathing<strong>The</strong> cough is spasmodic, the expectoration is exceedingly pr<strong>of</strong>use,<strong>of</strong> a frothy-serous consistence, sometimes pale and at other timesdark-red ; moist rfiles are heard in the whole chest ; cyanosis sitself first in the face and afterwards on the extremities.Gradually the sensation <strong>of</strong> suffocation increases in violence, theexpectoration becomes less, the rattling becomes louder and is beeven at a distance ; little by little, in consequence <strong>of</strong> the pois<strong>of</strong> the blood by carbonic acid, the patient becomes soporous, he ino longer able to cough up the sputa, because the muscular apparatus is semi-paralysed; the cough is less frequent, the breathinbecomes more and more superficial and shorter. Finally collapsesets in, the cheeks become livid, a rattling is heard in the tracand the patients die <strong>of</strong> asphyxia.K the oedema is extensive, and the air has been driven out <strong>of</strong> alarge portion <strong>of</strong> the lungs, the percussion-sound is muffled or typanitic ; as long as air remains in the oedematous portion, the pcussion-fiound remains unaltered. <strong>The</strong> local or circumscribed oedeon percussion frequently yields no alteration <strong>of</strong> the sound ; if tcells are entirely filled with serum, the percussion-sound may bedull and empty ; if situated at the apex <strong>of</strong> the lungs, this condi<strong>of</strong> the cells may be confounded with tuberculous infiltrations.
