We have already stated thatthe hemorrhage scarcely ever threatens life with immolate danger.158 Diseases <strong>of</strong> the Lungs«.Even copious and rapidly recurring hemorrhages destroy life onlyexceptionally. <strong>The</strong> result is difierent if the importance <strong>of</strong> thehemorrhage to the general organism is inquired inta <strong>The</strong> scantyexpectoration <strong>of</strong> blood which many persons, even without beingaffected with pneumonia, raise during every acute catarrh <strong>of</strong> therespiratory organs, is <strong>of</strong> very trifling significance, although itproper that a careful exploration <strong>of</strong> the chest should be institutwith a view <strong>of</strong> determining the presence or absence <strong>of</strong> tuberculardeposits. No more attention need be paid to the slight hemorrhages <strong>of</strong> pregnant or menostatic females, as long as no tuberculadeposits are at the foundation <strong>of</strong> the bleeding. Persons afflictedwith cardiac diseases, are very <strong>of</strong>ten attacked with pulmonaryhemorrhage which, however, does not imply any particular danger.In general, however, we have to admit that in by far the largermajority <strong>of</strong> cases pulmonary hemorrhage points to the presence <strong>of</strong>tubercles in the lungs and to the progress <strong>of</strong> the tubercular diseand that hence it is justly regarded as a dangerous symptom evenif a physical exploration <strong>of</strong> the chest does not yet reveal the prence <strong>of</strong> any alarming disorganization. If the hemorrhage is verypr<strong>of</strong>use, a return may indeed be apprehended, for a disposition tothe disease remains, and-oannot even be denied in cases where notubercles exist. <strong>The</strong> meaning and importance <strong>of</strong> pulmonary hemorrhage in cardiac affections will be examined more fully when wecome to treat <strong>of</strong> the affections <strong>of</strong> the heart.To these paragraphs we have to add a few remarks concerninginternal pulmonary hemorrhage, we mean infarctions and apoplexy,which frequently complicates pneumorrhagia and determines itsmost disastrous sequels.Infarctions in the lungs have their foci. At times there is one,at other times several, varying in size to that <strong>of</strong> a walnut and elarger. <strong>The</strong>y occur much less frequently near the periphery thanthe base <strong>of</strong> the lungs. Infarctions occur in the lungs as sharplydefined dark-red or black spots <strong>of</strong> a granular appearance on theircut surface, without much firmness ; a grumous, black fluid can bsqueezed out <strong>of</strong> them. <strong>The</strong> surrounding parts are either (edematousor inflamed. In real apoplexy the infarcted spot is always <strong>of</strong> morconsiderable size, and the pulmonary parenchyma is partiallydestroyed.Infarctions mostly take place where the pulmonary circulationhad met with considerable obstructions, hence in cardiac affectioless frequently in tuberculosis.
