Previous tothe acute attack setting in, the respiratory movements had becomevery much embarrassed, and these embarrassments are enhancedby the access <strong>of</strong> pneumonia, since the habitual expectoration <strong>of</strong>mucus is most commonly suspended as soon as the pneumonia commences. At the outset <strong>of</strong> such inflammations, if the fever is veryacute, -Bryonia and Mercurius are indispensable; Rhus will rarelyever appear suitable. Sometimes the symptoms indicate Tartaremetic from the start, or at any rate in one or two days ; in afftions <strong>of</strong> this kind, Tartar emetic <strong>of</strong>ten renders eminent service,vided the practitioner is not afraid <strong>of</strong> giving a sufficiently lardose. We have always found the second trituration necessary butsufficient. Verairum album deserves attention in such a case, espcially if marked cerebral symptoms are present, together with arapid sinking <strong>of</strong> strength and marked febrile motions, the pulsebeing at the same time very changing. Phosphorus^ which <strong>of</strong>tenseems indicated in such pneumonias, as well as Sulphur^ will scarever be <strong>of</strong> much use. In the further course <strong>of</strong> the disease, whetheit shows a very slow and gradually progressing improvement orelse an aggravation <strong>of</strong> the symptoms, Carbo veget. is a medicine ogreat importance which sometimes affords help even in desperate298 Diseases <strong>of</strong> the Lungs.cases. But not too mncli must be expected in a short time^becansethe course <strong>of</strong> the disease is always very much protracted. Lycopodium may come into play, although we cannot recommend itfrom i>ersonal experience. Nor have we derived much benefitfrom the use <strong>of</strong> Arsenicum which apparently seems indicated insuch cases ; at any rate it is much inferior to Carbo. <strong>The</strong> tardyabsorption <strong>of</strong> the infiltration is not specially accelerated by Suprobably because the conditions are wanting which favor the actio<strong>of</strong> this drug, namely a sufliciency <strong>of</strong> reactive power and a free a<strong>of</strong> the remaining respiratory channels ; Silicea^ on the contrary,motes the course <strong>of</strong> the disease in a striking manner. ' If the sytoms <strong>of</strong> passive hypenemia <strong>of</strong> the brain are very marked, the moatprominent remedy is Digitalis which may have to be given at thecommencement as well as at any other period in the course <strong>of</strong> thedisease ; it either removes or moderates the threatening symptoms<strong>of</strong> an imperfect return <strong>of</strong> blood from the brain.If pneumonia supervenes during whooping-cough, it is generallylobular, and as such has to be treated with the same remedies thahave been advised for the more malignant forms <strong>of</strong> this disease.Among them we mention more particularly Tartarus stibiatusyCuprum^ Mercurius^ Veratrum alburn^ Ipecacuanha. , Owing to theuncertainty <strong>of</strong> diagnosing this pathological complication, it is vdifficult to hit upon the most available remedy. No less malignanthan lobular pneumonia is croupous pneumonia, if supervening during whooping-cough. In such circumstances the course <strong>of</strong> the disease will scarcely ever be normal. Nervous phenomena set in at anearly period, the infiltration remains undissolved for a long timand a long illness may be expected without doubt, during whichthe lives <strong>of</strong> the children are in the greatest danger. In the casea child <strong>of</strong> eight months, which terminated favorably, Tartar emetihad the best eflTect, and next to it Veratrum and Ipecotmanhaywhereas Hepar sulphuris, Phosphorus, Sulphur, and other medicines apparently had not the least influence over the disease.
