Its conseqare: eccentric hypertrophy of the right ventricle and stasis of tblood in the veins of the systemic circulation. If the anomaly isconsiderable, it gives rise to venous pulsation. Defects of the mvalve and of the consequent excessive eccentric hypertrophy of thright ventricle may lead to a so-called relative insufficiency oftricuspid valve which is of special importance to a proper diagnoappreciation of the whole condition, for the reason that, on theappearance of this insufficiency, the stasis in the venous systemreaches an extraordinary degree, and dropsy is eminent. Thediagnosis is determined by a systolic murmur which is most distinctly audible in the region of the tricuspid valve. The treatmehas to be conducted in accordance with our remarks on mitralinsufficiency, since the consequences are entirely the same as thof the latter anomaly*The rest of valvular diseases as well as stenoses are exceedinglyrare; no special remarks can be offered regarding their treatmentO. ]f errons Palpitation of tbe Seart*Palpitatio Cordis.Palpitation of the heart, by which we mean a momentary, moreor less continuous increase of the frequency and strength of thebeats of the heart, is not only met with in a large number of morconditions beside the cardiac affections of which we have treatedin the preceding numbers, but likewise in persons of sound healthafter violent mental and bodily exertions. This is not the kind opalpitation that we wish to consider in this article, but thosedetached paroxysms of increased cardiac activity which set inwithout any demonstrable alterations in the substance of the hearand without any definite exciting cause. The etiology of this affection, as far as regards a knowledge ofthe physiological connection between cause and effect, is exceediobscure. All that we know is, that this palpitation frequently, b400 Diseases of the Heart.not by any means constantly, sets in after strong emotions eitherof an exciting or a depressing kind; that it may result from allsuch circumstances as weaken the nervous system and consequentlyrender it more irritable, such as anaemia, chlorosis, onanism, veexcesses, excessive mental exertions; that it may likewise resultfrom certain nervous afiections, such as hypochondria, hysteria, spinal irritation, softening ; that it may likewise result from tcondition which is designated as general plethora ; and that it fmay occur as a reflex etfect of certain morbid conditions, especiof the abdominal cavity, such as helminthiasis, cholelithiasis. The symptoms of this affection vary in the main in two directionsaccording as the increased action of the heart is only felt by thpatient, or is likewise perceptible as an objective phenomenon.
