Only a few pustubreak out at once, generally only one at first, the other pustulefollowing in successive crops. <strong>The</strong> eruption is sometimes precededby pains in the joints and febrile symptoms. <strong>The</strong> halo <strong>of</strong> the pustule fades away with the desiccation <strong>of</strong> the vesicle, and a firmlyadhering incrustation forms, not very thick, but which may increain thickness very considerably, if the suppurative process continunderneath it ; this change usually, however, takes place only incase <strong>of</strong> cachectic ecthyma. In consequence <strong>of</strong> a succession <strong>of</strong> cropthis pathological process may become protracted for weeks and evemonths. <strong>The</strong> pustules are most commonly seated on the legs, muchless frequently on the thighs and the rest <strong>of</strong> the body, scarcelyin the face.Rupia-pustules are large bullsB sometimes <strong>of</strong> the size <strong>of</strong> a modernpenny ; yet they ought to be ranged among the pustules for thereason that their contents though serous at first, yet soon changto a puriform liquid, and are very generally mixed with blood.<strong>The</strong>y are seated upon a dingy-red base, are never very muchelevated above the skin, but are loose blebs which, in a few dayschange to brown incrustations raised in the middle, and sometimesseparating very rapidly, leaving an ulcerated excoriation (rhypiasimplex), or beneath which the suppurative process continues andcauses the incrustations to grow to considerable thickness (rhypiprominens). If the incrustation is removed, a deep, sicklyulcer is revealed underneath which obstinately defies every attemto heal it, but upon which new crusts are disposed to form.Rupia, too, is most generally seated on the leg, never in the facvery seldom on the rest <strong>of</strong> the body. It ia an exanthem <strong>of</strong> a badcharacter, but is generally curable; it is only when the ulcerbecomes ichorous (rhypia gangrsenosa) that it may lea/1 to hecticfever and result in death.Pemphygus. 501Treatment. Ecthyma associated with scahies and the syphiliticrnpia will he treated <strong>of</strong> when we come to speak <strong>of</strong> the itch andsyphilis. If not accompanied hy malignant symptoms, ecthymahardly requires any medical treatment. Otherwise our best remedyis Antim. tartar.^ which is suitable as long as the cutaneous dishas not assumed a chronic character. Mercuriusis decidedly inferito Tartar Emetic. Arsenicum^ Staphysagria and perhaps Lt/cO"podium may be tried to keep <strong>of</strong>f the new crops <strong>of</strong> blebs. As longas rupia does not assume a gangrenous character, Shus tozicod.yAcidum nitricum^ muriaticum or phosphoricum are sufficient to curit; but if the affected parts are invaded by malignant ulcerationwe meet this change with excellent effect by means <strong>of</strong> Arsenicumand Secede comutum^ to which Carbo vcgetabilis and Staphysagriaare decidedly inferior although they may at times be useful. <strong>The</strong>main object is to treat the general condition ; both exanthems widisappear so much sooner and will be so much less disposed to breout again, the more carefully the skin is attended to, together wthe internal treatment.19.
