If the compensation becomes insufficient,Bymptoms <strong>of</strong> a deficient supply <strong>of</strong> blood to the brain and <strong>of</strong> realanaemia first become apparent, after which symptoms <strong>of</strong> stasis inthe lesser circulation and in the veins slowly show themselves.898 Diseases <strong>of</strong> the Heart.It is impossible to indicate a definite mode <strong>of</strong> treating thisstenosis, for the simple reason that the character <strong>of</strong> this anomalvaries too much and that it is always complicated with otherabnormal alterations. As long as the patients feel well, medicineis useless or even hurtful; the stenosis cannot be cured and allthe medicine might possibly accomplish, would be to disturb thefunctional cardiac equilibrium. If the compensation becomes insufficient, we have to resort to the remedies that were recommendfor fatty degeneration: Veratrum^ lodium^ Arsenicum and especiallDigitalis^ and with reference to the deficiency in the process <strong>of</strong>sanguification: Ferrum^ Calcarea carbonica and China. If symptoms <strong>of</strong> venous stasis make their appearance, we have to choosesuch remedies as have been reconmiended for mitral stenosis. <strong>The</strong>regimen to be pursued differs <strong>of</strong> course from that recommendedfor aortic insufficiency, since in view <strong>of</strong> the deficient supply oarterial blood the process <strong>of</strong> assimilation is <strong>of</strong> course rendereddifficult. Hence our chief aim should be to give the body a chancto make good blood, by furnishing it a supply <strong>of</strong> easily digestedand nourishing food and an abundance <strong>of</strong> fresh air. Even the use<strong>of</strong> wine need not be forbidden; on the contrary, it is <strong>of</strong>ten to berecommended, both in order to promote the process <strong>of</strong> digestionand as a direct stimulant for the heart. If the stenosis is at thsame time associated with insufficiency <strong>of</strong> the semilunar valves,the treatment becomes much more difficult, because it is <strong>of</strong>ten dicult to decide, the consequences <strong>of</strong> which <strong>of</strong> these two anomaliesought to be met first. Generally one anomaly wnll probably bemore marked than the other, in which case the course to bepursued in the treatment is naturally indicated.<strong>The</strong> combinations <strong>of</strong> valvular diseases <strong>of</strong> the left heart, as theygenerally present themselves, complicate the treatment by presentdifficulties for which it is, however, impossible to arrange defitherapeutic rules and practical applications <strong>of</strong> drugs. Since condtions <strong>of</strong> this kind fortunately never require the immediate intervention <strong>of</strong> remedial agents, there will always be time enough in agiven case to consider what treatment had better be pursued, andwhat remedies are most adapted to the case.
