In very rare cases there is no cough during thewhole coarse <strong>of</strong> the disease, although the infiltration may be extsive and numerous caverns may exist.Previous to the deposition <strong>of</strong> numerous tubercles and their purulent dissolution, the expectoration is without^ any characteristifeatures. K'evertheless a continual expectoration <strong>of</strong> clear mucusalways a suspicious symptom ; but such an expectoration acquiressignificance when it is traversed by fine streaks <strong>of</strong> a yellowishand when, moreover, it frequently appears streaked with blood.Still more characteristic is the presence <strong>of</strong> sago-like firm littland fibres. Much less frequently hard, yellow or greenish, cheesylittle lumps are coughed up at an early period; they generally tavery badly, and frequently have an <strong>of</strong>fensive odor. Sometimes theexpectoration contains small lumps <strong>of</strong> calcareous matter, chalktubercles, seldom, however, previous to suppuration having fullyset in ; usually after caverns have formed. <strong>The</strong> cavernous expectoration has a yellow-gray color, less frequently a greenish color,firmly coherent; in the water it forms irregular, shaggy ballswhich slowly sink to #he bottom ; it is surrounded by a quantity<strong>of</strong> clear bronchial mucus with which, however, it does not mix.<strong>The</strong> microscope discovers elastic fibres in this expectoration, asign <strong>of</strong> existing phthisis.<strong>The</strong> larynx and trachea seldom remain intact during the wholecourse <strong>of</strong> the disease ; they show more or less marked symptoms<strong>of</strong> catarrhal irritation. At an early period <strong>of</strong> the disease, the vbecomes husky or loses its resonnance and firmness <strong>of</strong> tone; at alater period, these organs become painful, according as the abovedescribed ulcers are located, and not unfrequently complete aphonsets in.<strong>The</strong> phenomena <strong>of</strong> the circulatory organs are not by any meansconstant, but at the beginning <strong>of</strong> tuberculosis we generally finddilatation <strong>of</strong> the right heart, disposition to a tumultuous actionthe heart, blowing murmurs under the clavicle, anaamic mummrsin the jugulars, continuation <strong>of</strong> the sounds <strong>of</strong> the heart as far aboth apices <strong>of</strong> the lungs. Afterwards the heart becomes more andmore atrophied. <strong>The</strong> tendency to sudden changes <strong>of</strong> color is astriking phenomenon only at the commencement <strong>of</strong> the disease.Very soon the pulse becomes frequent and feebler than usual, it iaccelerated by every movement. In the stage <strong>of</strong> phthisis it almostalways exceeds 100, sometimes rising to 140. It is only exceptionally that the pulse <strong>of</strong> phthisicky patients is retarded, and thenTuberculosis* 667occasionally and alternating with a frequent pulse.
