If diffugangrene sets in without pneumonia, we perceive from the startsymptoms <strong>of</strong> adynamia resembling the phenomena that generallyaccompany septicaemia with chills, stupor, delirium, singultus, eCircumscribed gangrene sometimes cannot be recognized untilthe gangrenous ichor has passed into the bronchia and is expelledby the mouth.Some patients bear this severe disease without much distress;their constitutional condition is not very much disturbed; theyhave no fever, walk about, attend to some light, domestic employment, and the disease <strong>of</strong>ten drags along for months.If the gangrened portion <strong>of</strong> lung does not communicate with abronchus, and has become isolated as within a capsule in consequence <strong>of</strong> secondary inflammation, the cadaverous odor may bealtogether wanting.To infer the presence <strong>of</strong> pulmonary gangrene alone from thecadaverous odor, would be an exceedingly superficial mode <strong>of</strong>reasoning, since this symptom may likewise occur in bronchiectasiand when tubercular caverns are present.<strong>The</strong> physical signs during this morbid process are not veryimportant.In diffuse pulmonary gangrene consequent upon pneumonia, percussion at first yields a dull or tympanitic sound; auscultationreveals indistinct respiratory murmurs or rales ; in the subsequecourse <strong>of</strong> the disease percussion reveals cavities in the lungs, tsame as in the case <strong>of</strong> tubercular caverns, and auscultation reveabronchial respiration and cavernous sounds.Circumscribed gangrene is very difficult to recognize by physicalexploration.Course, Terminafian, Pi^ognoHts.
