It is true that incases <strong>of</strong> acute pleuritis we have the peculiar pain, but it variesexceedingly in character and intensity. At times the pain is simpa sensation <strong>of</strong> tenseness and constriction ; at other times the paa seated, localized, stitching pain ; at other times again the paspread over the whole side <strong>of</strong> the thorax, and is <strong>of</strong> a tearing orburning kind. Not unfrequently there is no pain at all ; this hapmore particularly in the most insidious cases. Because there is npain, this is no reason why an exploration <strong>of</strong> the chest should beomitted. In genuine pleurisy there is very seldom any cough,especially at the commencement <strong>of</strong> the disease ; if cough sets ina later period, it may be occasioned by simple hypersemia, or bypneumonic infiltration. At all events it causes the patient a goodeal <strong>of</strong> distress, increases his pain to an extraordinary degree acomplicates the course <strong>of</strong> the disease, since every additional obsmust necessarily, in view <strong>of</strong> the existing dyspnoea, imply an additioDal amount <strong>of</strong> danger. <strong>The</strong> symptoms presented by other organscanoot be determined beforehand. Most generally it is the action<strong>of</strong> the heart, especially that <strong>of</strong> the right heart, which is altereif any considerable amount <strong>of</strong> exudation is present, the beats <strong>of</strong>heart become irregular, stronger and more rapid, the more so themore the heart is pushed out <strong>of</strong> its place by the effused fluid. Neither theintestinal canal nor the kidneys are involved in all cases. <strong>The</strong> bbecomes slightly engorged, if the effusion is copious and the cirlation is interfered with. <strong>The</strong> position <strong>of</strong> the patients deservesparticular notice, more especially as contrasted with their positin pneumonia. Pleuiitic patients almost always lie on the painlesand unaffected side, whereas, if the effusion is not attended witpain, they preier lying on the diseased side.
