Years ago Fleischmann showed that typhusgenerally runs a favorable course under the influence <strong>of</strong> Arsenicuand we should think that several hundred cures ought to be something in its favor. Fleischmann, however, generalizes too muchwhereas Wurmb, on the other hand, errs in individualizingbeyond necessity. After all, the balance inclines in Fleischmann'favor, for Arsen. produces the symptoms <strong>of</strong> lentescent as well asinflammatory and putrid typhus. Our advice therefore is that Arseshould be given more frequently than has been customary, fromthe very beginning <strong>of</strong> the attack, and that we should not waituntil the disease has fully developed its pernicious character. Afew special indications for Arsen. are the following : Striking atypical remission <strong>of</strong> the fever, having the appearance <strong>of</strong> an actuaintermission. Marked meteorism <strong>of</strong> the bowels, with gurgling in thbowels, but no diarrhcea. Extensive hypostasis <strong>of</strong> the lungs, withbronchitis and considerable hoarseness. Irregular action <strong>of</strong> theheart, absence <strong>of</strong> the second sound <strong>of</strong> the heart. Paroxysms <strong>of</strong>sudden collapse towards midnight. Fetid breath. Frequent retching and even vomiting, immediately after drinking, in the secondand the subsequent weeks or during convalescence. Considerableenlargement <strong>of</strong> the spleen. In exan thematic typhus Arsen. is indicated much less frequently, and is chiefly adapted only to the puform, the so-called putrid typhus. In lentescent typhus Arsen. isan excellent remedy, if the symptoms exacerbate every other day;11" the patients are tormented by anxiety ; if every little meal38594 Epidemic and Endemic Infectious Diseases.retching and vomiting, although the tongue is clean. Arsen. is somuch more extensively applicable the more typhus assumes theepidemic form ; likewise in endemic typhus. Most observers advocate repeated doses <strong>of</strong> the second to the fourth trituration. [Weremember a case <strong>of</strong> exanthematic typhus where the putrid stagehad fuUy set in ; the patient was emaciated and extremely prostrated ; the teeth looked black ; the tongue was swollen, blackisand bleeding ; blood oozed from the gums ; the skin was coveredwith petechise, from many <strong>of</strong> which a black, foul, decomposed bloowas constantly oozing ; involuntary, bloody, horridly-smelling stA few globules <strong>of</strong> Arsen. 18th excited a reaction which ended inperfect recovery in one week. H.]Phosphorus is one <strong>of</strong> the most important remedies in typhus ; itscurative action is, however, limited to certain sharply defined cit cannot be regarded as a general remedy for typhus, in the samesense as Arsen. or Bryon. — Above all, Pkosphoms is indicated inpneumo-typhus with violent bronchitis, hepatization, together withypostasis and laryngitis. It is a sovereign remedy for this condtion, unsurpassed by any other medicine. It is indicated by thefollowing abdominal symptoms: Frequent diarrhoeic stools at anearly period <strong>of</strong> the disease, coming on after every meal, <strong>of</strong> a dardingy, blackish-gray color, or mixed with decomposed blood andcontaining shreds <strong>of</strong> intestinal mucous lining. Extreme prostratioafter every discharge. Numerous roseola-spots, with ecchymosesand sudamina.
