On the part<strong>of</strong> the body most remote from the heart, the skin generally losesturgescence, its temperature falls quite considerably, its colorbluish and as if dead. On the other hand, the temperature in theIntermittent Fever. 665axilte as well as in the mouth is considerably higher. <strong>The</strong> chillvery generally accompanied by violent headache and a distressingthirst, and, if any food had been introduced into the stomachshortly before the paroxysm, it is ejected when the chill sets in<strong>The</strong> chill seldom lasts less than half an hour, nor more than threhours. <strong>The</strong> pulse is small and contracted, the urine has not muchcolor, the spleen is somewhat enlarged.<strong>The</strong> second or hot stage never sets in all at once ; the chill abagradually and is interrupted by local flashes <strong>of</strong> heat, until thefinally becomes permanent and general. With the heat, the turgescence <strong>of</strong> the skin likewise returns, the temperature graduallyincreases to a considerable height, sometimes over 100° F., and tpulse becomes full and bounding. <strong>The</strong> headache, restlessness andthirst increase considerably, delirium is not unfrequent, the col<strong>of</strong> the face changes to a bright-red. <strong>The</strong> enlargement <strong>of</strong> the spleecontinues during this stage which seldom lasts less than two tothree, but frequently upwards <strong>of</strong> six or eight hours.<strong>The</strong> sweaty stage likewise sets in gradually; perspiration firstbresiks out on some parts and gradually covere the whole body. Atthe same time the temperature sinks rapidly, according to Wunderlich, in regular stages, and a feeling <strong>of</strong> comfort is soon restorethe patient. As a rule, the urine at this stage is strongly saturwith urates. <strong>The</strong> sweat lasts from one to six and even ten hours ;the apyrexia commences as soon as the sweat ceases. This periodwhich is <strong>of</strong> the utmost importance to a homoeopathic practitioner,always shows some, although sometimes very trifling morbid symptoms which differ in diiFerent individuals.Fever-and-agne, in its uncomplicated form, generally follows thetertian type ; in other words, forty-eight hours elapse between tsuccessive paroxysms. If the intensity <strong>of</strong> the fever remains thesame, the paroxysms generally return at the same period. In proportion as the disease decreases in intensity, the next paroxysmalways sets in at a somewhat earlier period, and in a less degreeintensity ; it is much less frequently the case for the paroxysmset in with less force and at a later period. If the paroxysm setin sooner and with more violence, a transformation <strong>of</strong> the tertianinto the quotidian type may be expected ; a change <strong>of</strong> this kindalmost always takes place, if the fever lasts a good while.
