Mastitis is most a96 Diseases <strong>of</strong> the Female Sexual Organs.to occur soon after confinement or shortly after weaning the chilmastitis occurring at the latter period is less apt to lead to thmation <strong>of</strong> abscesses.Symptoms. <strong>The</strong> disease never breaks out all at once. Generally women experience some time previous, a gradually increasingpain both spontaneously or while the child is nursing ; and a swe<strong>of</strong> one or more mammary lobules, which rapidly increases in extentand induration, develops itself soon after. As a rule the lower olateral lobes are inflamed, very seldom the upper ones, and stillless frequently both mammae. Sooner or later, sometimes in a fewdays, and at other times in some weeks, the painful spot becomesred and more sensitive, and the inflammation is intense, the wholorganism feels the effect <strong>of</strong> the inflammatory process and shows isympathy by febrile phenomena. Soon the infiltrated tissue showsthe signs <strong>of</strong> suppuration, the pus being discharged through a smalopening. As soon as the discharge, which is never complete, commences, the pain suddenly ceases and the febrile symptoms abate.<strong>The</strong> course <strong>of</strong> the inflammation depends upon a variety <strong>of</strong> circumstances. If the inflammation is confined to one lobe, the abscmost commonly discharges close to the nipple, empties itself veryrapidly and heals. If several lobes are inflamed, or the inflamedlobe is situated near the base, the suppuration, swelling and paicontinue for a long time ; months and even years may elapse beforthe abscess heals, which discharges through several openings nearthe nipple. This disorder never lasts less than two weeks, and,under corresponding circumstances, may continue for months evenin its acute form.<strong>The</strong> treatment <strong>of</strong> mastitis includes above everything else a prophylactic treatment which has to be commenced long before confinement, especially in the case <strong>of</strong> primiparse. <strong>The</strong> preservation andproper management <strong>of</strong> the nipples and <strong>of</strong> the mamm» generallyshould be a constant subject <strong>of</strong> our attention. After confinement,if women do not intend to nurse their children, they will have todispense with liquids as much as possible. Nursing women have tokeep the following points constantly in view : the breasts must nbe kept too warm, must not be enclosed in tight clothing, and musby some suitable support, be prevented from dragging down ; everytime after the baby has nursed, the nipple must be washed, but nokept moist between the acts <strong>of</strong> nursing ; the excessive use <strong>of</strong> liqwhich women fancy favor the secretion <strong>of</strong> milk, must be avoided ;the child should be put to the breast as seldom as possible, forMastitis. 97more hungry it is the more vigorously it will draw and the morecompletely the breast will be emptied.
