This circumstance isin some respects accounted for by the patient's more luxurious mo<strong>of</strong> living during the winter-season, when he takes at the same timless active exercise than usual. <strong>The</strong> interval is entirely free frailments until shortly previous to the new attack, when the formepremonitory symptoms again make their appearance.With every new attack the free intervals are more and moreobscured and shortened, the local disorder leaves calculous deposon the diseased joint, with more or less permanent pains and a hior less degree <strong>of</strong> immobility. <strong>The</strong> paroxysms are less acute, butinstead <strong>of</strong> at most two, they last eight or twelve weeks; otherjoints, especially those <strong>of</strong> the feet, knees, hands and shoulders,wise become involved. <strong>The</strong> so-called chronic or atonic gout now isan established infirmity. <strong>The</strong> more numerous the changes thatremain after the attack, the more joints are affected, the longerattack lasts, the more prominently the chronic affection shows itcharacteristic features during the interval. <strong>The</strong> digestion is permanently deranged, the patients complain <strong>of</strong> pains in the stomach,which sometimes increase to severe paroxysms <strong>of</strong> cardialgia ; theycomplain, moreover, <strong>of</strong> acidity <strong>of</strong> the stomach, pressure, fulnessand flatulence after every meal, torpor <strong>of</strong> the bowels and hemorrhoidal distress ; they are very irritable and out <strong>of</strong> humor. Nutrtion is more and more impaired, the complexion assumes a yellowgrayish appearance. <strong>The</strong> circulation is disturbed in various ways.Such disturbances are very significant in so far as they generallpoint to a degeneration <strong>of</strong> the valves and arteries ; they commencwith frequent, but short-lasting congestions, palpitations, attac<strong>of</strong> angina pectoris, asthmatic complaints. <strong>The</strong> affected joints become more and more deformed and anchylosed.At a more advanced age and by observing a strict diet, thischronic gout may terminate in recovery; <strong>of</strong> course, the concretionin the joints remain. This form <strong>of</strong> gout may likewise assume thecharacter <strong>of</strong> the so-called anomalous gouty which sometimes ushersin the whole disease ; in such a case it becomes exceedingly diflto establish a reliable diagnosis. Anomalous gout may likewisesucceed the first attack <strong>of</strong> gout in the place <strong>of</strong> chronic gout. Thpatients suffer with the complaints <strong>of</strong> chronic gout ; above all,urine looks very cloudy depositing large quantities <strong>of</strong> urates ; bin the place <strong>of</strong> acute pains in the joints, we have shortshifting about from place to place frequently and speedily, withoArthritis. 643swelling or redness, "but very frequently with gradual impairment<strong>of</strong> the mobility <strong>of</strong> the joints. Of particular importance are theintermediate affections <strong>of</strong> internal organs that have been designaas retrograde or metastatic gout.
