We have to examine these pointsmore fully, because upon their correct knowledge and appreciationdepends the possibility <strong>of</strong> preserving the lives <strong>of</strong> both mother anchild.In the first three months, hemorrhage is generally the first symptom <strong>of</strong> incipient miscarriage. As a rule, the hemorrhage at onceassumes the form <strong>of</strong> metrorrhagia, even if the woman had beenotherwise perfectly healthy, especially if violent external impresions were, the cause <strong>of</strong> the accident. A mere dribbling <strong>of</strong> bloodthis time is <strong>of</strong> much less frequent occurrence. At first the patiedoes not complain <strong>of</strong> any pain ; after a short lapse <strong>of</strong> time, it sin very gradually as a dull pain, and increases until it finallysumes the character <strong>of</strong> real labor-pains ; this is likewise substatiated by the circumstance that, during the paroxysms <strong>of</strong> pain, thblood is expelled in larger quantity. <strong>The</strong>se phenomena are mistaken by inexperienced women, if they are otherwise afflicted witdifficult and pr<strong>of</strong>use menstruation, for a return <strong>of</strong> the menses.In the subsequent stages <strong>of</strong> pregnancy, metrorrhagia only takesplace exceptionally, without a preliminary stage which is determined by the entrance <strong>of</strong> blood into the uterus whose mouth stillremains closed ; or by the premature contractions <strong>of</strong> this organ,by the death <strong>of</strong> the foetus.In the first <strong>of</strong> these three cases, the patients complain <strong>of</strong> a dulpain in the pelvis which extends to the back and is increased bymotion, by voiding the urine or by evacuating the bowels. <strong>The</strong>symptoms are frequently associated with slight febrile motions,diarrhoea with tenesmus, urinary difficulties. All these phenomenrarely precede the hemorrhage longer than five days. It commenceswith distinct labor-pains.<strong>The</strong> premature contractions <strong>of</strong> the uterus may be allied to thecongestive phenomena just described, but they likewise occur asprimary symptoms. In such a case the patient experiences a peculiar drawing pain from the uterus across the abdomen, and spreading with peculiar violence to the small <strong>of</strong> the back, at first intmitting for hours, and finally only for a few minutes. <strong>The</strong> generaorganism does not seem to be affected by this pain. Sometimes thepains intermit even for days ; nevertheless, they return again.
