In the efflorescences occasioned by the young acari, one<strong>of</strong> them is seldom found for the reason that they wander aboutfrom place to place very rapidly. On individuals who do not washthemselves with care, a cuniculus is most easily discovered by lipassing a finger moistened with saliva over it several times, insequences <strong>of</strong> which the dirt penetrates between and adheres to thescales <strong>of</strong> the epidermis. In persons with callous hands the acarushas to be sought at the wrist.A transmission <strong>of</strong> the acarus is most easily effected when it hasthe greatest desire to wander, hence in a warm bed ; it takes plamuch less frequently by touching objects that had been handledby persons affected with the itch; but it is easily accomplishedthrough wearing apparel.<strong>The</strong> phenomena caused by the acarus upon the skin, proceed fromit either directly or indirectly. <strong>The</strong> presence <strong>of</strong> the acarus uponthe skin remains without symptoms except a slight itching whichis not always present, and does not always become very troublesome even when, as is <strong>of</strong>ten the case, especially on the penis, thcuniculus is closed by a little exudation deposited below it andits side. But immediately after leaving the canal, the young acaragain bury themselves in its proximity, and small papules toppedwith fine vesicles break out close to the primary canal, itchingviolently, and being either speedily lacerated by scratching or,in the case <strong>of</strong> children, changing to pustules and forming scabs acrusts <strong>of</strong> various sizes. <strong>The</strong> indirect phenomena, according toHebra, owe their origin entirely to the scratching caused by theitching ; as an evidence <strong>of</strong> this fact he refers to the circumstanthat children and paralytic persons who do not scratch, remain frfrom these symptoms, and that in healthy persons they are onlymet with in localities that can be scratched. First papulee, eachperforated by a little hair, break out, less frequently vesiclespustules. <strong>The</strong>se papules are scratched, after which their tips become covered with fine, dark-looking scabs. After the cutaneousirritation has lasted for some time, pustules arise from the papuOn parts <strong>of</strong> the body that are exposed to constant pressure, or toconstant pressure with friction, the skin soon appears infiltrateand is thickly covered with the above-mentioned efflorescences, afor instance round the waist in the case <strong>of</strong> females on account <strong>of</strong>the clothes being fastened around too tightly, or in the case <strong>of</strong>510 Diseases <strong>of</strong> the Skin.Bhoemakers on the nates.
