"No more than we were guided in oarfirst trials with Hepar in pneumonia by the few correspondingsymptoms <strong>of</strong> this drug, has Wurmb been guided by the symptomsalo^e when he says <strong>of</strong> Hepar: "To my knowledge, Hepar has neverbeen employed in pleuritis, yet few remedies render such eminentservice in this disease as Hepar. If the effusion is rather <strong>of</strong> aplastic nature, and if the disease has lasted for some time, or islow, lentescent course is to be apprehended from the commencement <strong>of</strong> the attack, I know <strong>of</strong> no better remedy than Hepar.Under its use I have seen copious exudations, which had already eisted for a long time, disappear in a comparatively and incrediblshort period <strong>of</strong> time, nor have I ever observed a case where, undesuch circumstances, it did not exert some influence upon the exudation. A complication with pericarditis or bronchitis demands itexhibition so much more urgently ; if the effusion is rather moreserous, Hepar is <strong>of</strong> little use." We share this opinion, adding atthe same time that empyema, provided it is not too old, is aboveall calculated to substantiate the favorable action <strong>of</strong> this drug,that the evident passage from the sero-plastic to the purulent fo<strong>of</strong> pleuritis points more especially to Hepar.Squilla maritima is much more talked about than actually used inpleuritis. We believe, however, unjustly, for the physiologicaleffects <strong>of</strong> this drug show that it is in special rapport with thepleura and likewise with the lungs. Squilla is decidedly one <strong>of</strong>the medicines that it is proper to try in the treatment <strong>of</strong> pleuriIt is more particularly adapted to pleuro-pneumonia, and is moreespecially indicated by a copious exudation <strong>of</strong> serum and even inserous pleuritis where the curative action <strong>of</strong> this drug manifestsitself by a pr<strong>of</strong>use diuresis. In former times Squills enjoyed agreat reputation in this disease, and have fallen into disuse inold system <strong>of</strong> medicine probably for no other reason than becausethe medicine did not help in every case, because its more specialindications were either not sought or physicians did not know hewto seek them.We do not regard Pfiosphorus as a direct remedy for pleuritisalthough, having been mentioned as a remedy for pneumonia, it isvery likely that it may be <strong>of</strong> some use in peripneumonia, in whichcase, however, the pleuritic exudation will be very scanty.
