Death takes place witsymptoms <strong>of</strong> paralysis <strong>of</strong> the heart and lungs. In favorable casesthe fever remits every morning on the last days <strong>of</strong> the third weekwhereas the evening-exacerbations decrease in violence, the consciousness returns gradually and with it a desire for food and drAt this stage the fever scarcely ever shows a sudden and considerable decrease, with a correspondingly sudden beginning <strong>of</strong> convalescence.With the third week typhus <strong>of</strong> a medium grade has reached itsintensity in so far as signs <strong>of</strong> convalescence now begin to showthemselves, although very slowly. Every case <strong>of</strong> typhus outing the third week, may be safely regarded as very severe. Exceptcomplications take place, no new symptoms develop themselvesduring the fourth and fifth weeks. <strong>The</strong> remissions <strong>of</strong> the feverare very distinct and grow more decided in character; the pulse,on the contrary, increases in weakness and frequency. <strong>The</strong> patientlooks as if he were in a state <strong>of</strong> terrible collapse; occasionallythreatening paroxysms <strong>of</strong> collapse really take place, more particularly in the night ; in the later course <strong>of</strong> the disease, such paysms are, however, not as dangerous as they seem. In the case <strong>of</strong>adults the consciousness usually returns towards the end <strong>of</strong> thefourth week; but, if the fever continues, delirium is still veryfrequent towards evening. <strong>The</strong> diarrhoea is at times less than itwas, and at other times more pr<strong>of</strong>use ; in some cases it now ceasealtogether. <strong>The</strong> skin looks clean, with the exception <strong>of</strong> miliariahere and there. <strong>The</strong> bed-sores which are never wanting duringsuch a protracted course <strong>of</strong> the disease, grow rapidly in extent amalignancy.If the fever runs a course <strong>of</strong> four weeks, recovery takes placevery slowly. In very fortunate cases recovery goes on uninterruptedly, but most frequently its course is disturbed by all sortaccidental inconveniences, such as vomiting after certain kinds <strong>of</strong>ood, or even after any kind ; sudden disappearance <strong>of</strong> the appetithat had just begun to return; return or protracted continuance<strong>of</strong> the diarrhoea; exacerbations <strong>of</strong> the fever. <strong>The</strong> decubitus, thenature and conduct <strong>of</strong> which aftbrd an excellent criterium by whicthe amount <strong>of</strong> progress in the recovery <strong>of</strong> the patient can bemeasured, sometimes causes a great deal <strong>of</strong> serious trouble, provithe final cure <strong>of</strong> the fever is very much delayed. Death eithertakes place in consequence <strong>of</strong> the utter prostration <strong>of</strong> the patiejTyphus. 583^r else is caused by the complications and sequelse that will bementioned hereafter. Before, however, dwelling upon them, wehave to mention various frequently occurring deviations from thegeneral course <strong>of</strong> the disease. It ought to be observed, however,that typhus varies in its manifestations more than any other disease, sometimes to such an extent that it is only with great diffculty that the attack can be diagnosed as typhus.Abortive typhus may terminate in convalescence in the first orsecond week.
