In the case <strong>of</strong> children themucous coat <strong>of</strong> the stomach, especially in the region <strong>of</strong> the culsac, is found transformed into a gelatinous, colorless pulp whichm the case <strong>of</strong> adults, has a dirty-brown color. <strong>The</strong>re are no signs<strong>of</strong> gastritis or ulceration. During the formative stage <strong>of</strong> the s<strong>of</strong>ing the mucous coat is interstitially distended, s<strong>of</strong>t and lax ; ieasily destroyed by scraping it with the back <strong>of</strong> the scalpel andcan no longer, as in its normal condition, be detached in patchesfrom the submucous cellular tissue. In a high degree <strong>of</strong> s<strong>of</strong>teningall the coats <strong>of</strong> the stomach decay and the shape <strong>of</strong> the stomach isustained only by the thin, gauzy peritoneum which tears by theslightest touch; or else the stomach is perforated and its contenare poured into the abdominal cavity or, if the oesophagus is involved in the s<strong>of</strong>tening, into the thorax, more especially into itleft half. If the serous coat is intact, the stomach is mostly ditended with gas and chiefly contains a badly-colored, sourfluid. <strong>The</strong> s<strong>of</strong>tened parts are never sharply circumscribed; thes<strong>of</strong>tening always commences in the mucous coat whence it spreadsoutwardly, frequently drawing the (Bsophagus, the spleen and theleft half <strong>of</strong> the diaphragm into the morbid process.Etiology. Gastromalacia most commonly attacks children <strong>of</strong>a year old, whose constitutions have become deteriorated by improper nourishment, by disease or general neglect. It most frequently occurs after weaning.Secondarily it is apt to set in as a sequel <strong>of</strong> cholera infantum ;sometimes it develops itself in the course <strong>of</strong> hydrocephalus andmeningitis.In the case <strong>of</strong> adults, gastromalacia always constitutes a secondary process developing itself in the course <strong>of</strong> malignant typhus,pyeemia, puerperal fever, acute tuberculosis, tubercular meningitis, etc<strong>The</strong> circumstance that s<strong>of</strong>tening <strong>of</strong> the stomach has been metwith in the case <strong>of</strong> perfectly healthy individuals who suddenly dishortly after a meal, and that the stomachs <strong>of</strong> infants at the bre47 (737J738 Gastrotnalacia.are always found in a state <strong>of</strong> s<strong>of</strong>tening if they contain milk shoafter the infants' death, has led modern pathologists to assert ts<strong>of</strong>tening <strong>of</strong> the stomach is under all circumstances a simple postmortem appearance.
