Full text <strong>of</strong> "<strong>The</strong> <strong>Science</strong> <strong>of</strong> <strong>The</strong>rapeutics: According to the Principles <strong>of</strong> Homeopath...Page 461 <strong>of</strong> 653chronic buboes.<strong>The</strong> acute bubo seldom develops itself in the second to the sixthweek after the appearance <strong>of</strong> the primary ulcer, seldom at an earlperiod, and likewise seldom after the cicatrization <strong>of</strong> the ulcer.generally breaks out after simple chancre, and is most likely tomake its appearance if the chancre is located close to the frenulIrritating the ulcer, most likely favors the development <strong>of</strong> a bubAt a spot in the bend <strong>of</strong> the groin, the patient, especially wheamoving the part briskly, experiences a pain which is very muchaggravated during contact, although the inflamed gland can scarceyet be felt. It generally grows rapidly in size, is very painfulmay reach the size <strong>of</strong> a goose-egg. It adheres to the integuments,hence is not moveable, and the integuments soon show a red color.Suppuration does not take place very rapidly ; fluctuation is nevperceived over a large surface, because resolution only takes plafragmentarily. <strong>The</strong> spontaneous breaking <strong>of</strong> the abscess takesplace with much dilEculty ; after its discharge the truly virulenbubo forms an ulcer with all the characteristics <strong>of</strong> chancre, thesecretion from which is infectious. A bubo very seldom arises froa simple sympathetic irritation <strong>of</strong> the lymphatic gland. In thelatter case, a cure mostly takes place very speedily, whereas informer case the healing process is very tardy, and the swelling mremain even a long time after the wound is closed. An extension<strong>of</strong> the inflammatory process to the surrounding cellular tissue, eCO the peritonaeum ; the formation <strong>of</strong> fistulous canals and a descsion <strong>of</strong> the pus are not unfrequent occurrences. <strong>The</strong>se bad resultsgenerally occur if the bubo is lanced prematurely. We shall reverto this subject when we come to speak <strong>of</strong> the treatment <strong>of</strong> syphili<strong>The</strong>re is no difllculty in diagnosing the true nature <strong>of</strong> a bubo, ecept after the chancre is healed without leaving a scar, or if thbubo develops itself without any previous chancre. A characteristcircumstance in the history <strong>of</strong> a syphilitic bubo is, that it almoalways breaks out below Poupart's ligament.Syphilis. 647<strong>The</strong> chronic, indolent bubo runs a slow course ; it comes on without pain and is very obstinate ; it is distinguished from an acutbubo by the circumstance that it only occurs in connection with aindurated chancre, and is not, like an acute bubo, confined mostlto one gland, but involves a number <strong>of</strong> them. <strong>The</strong> swelling progresses very slowly ; a single gland does not attain any considersize, but from several glands swelling close together, a large, hbunchy tumor is formed. Single glands very seldom terminate insuppuration ; if this happens, pus forms very slowly, the sides othe abscess are very slack, and a whole gland is transformed intopurulent matter. Very seldom the inflammation attacks the surrounding parts. Whereas an acute bubo does not justify theconclusion that secondary phenomena will certainly make theirappearance, an indolent bubo is almost invariably succeeded bythem. ^A catarrh <strong>of</strong> the mucous lining <strong>of</strong> the urethra or vagina does notvery frequently accompany the primary ulcer. Chancres in theurethra may run their course without a sign <strong>of</strong> gonorrhoea, or elsthe gonorrhoea may be very violent and painful.
